克服安大略省乡村地区变性人获得医疗保健服务的障碍——关于个人代理权、复原力和赋权的话语

Overcoming the barriers in transgender healthcare in rural Ontario: discourses of personal agency, resilience, and empowerment

摘要 本项研究从定性方面探讨变性人在安大略省乡村医疗保健环境中遇到的障碍,分析了变性人如何克服这些障碍,以及如何加强医疗互动来提升他们个人代理体验的方法。安大略省乡村地区的12名变性者和3名医疗专业人士参与了这项开放式、半结构化的访谈,对访谈内容进行解释现象学和批评话语分析后,让我们深刻认识到了医疗保健互动中侵犯人权,安大略省变性病人在农村面临的挑战,非互惠关系对变性人个体的自我代理、自主性和心理健康体验等方面的影响,以及实现他们整体复原力和赋权的有利因素等问题,还探讨了变性人医疗保健的可行模式和未来研究方向等问题。 This study qualitatively explores the barriers that transgender individuals experience within healthcare settings in rural Ontario.It includes an analysis of how transgender individuals overcome these barriers,as well as the methods by which healthcare interactions can be strengthened to increase their experiences of personal agency.Twelve transgender individuals and three healthcare professionals in rural Ontario participated in open-ended,semi-structured interviews,which were analysed using interpretive phenomenological analysis and critical discourse analysis.Together these analyses yielded insights on themes pertaining to the breaching of human rights in healthcare interactions;rural challenges experienced by transgender patients in Ontario;the effects of non-reciprocal relationships on trans-patients’experiences of personal agency,autonomy,and mental health;and factors contributing to their overall resiliency and empowerment.Potential transgender healthcare models and directions for future research are also discussed.
