血脂控制水平与冠状动脉内支架术后再狭窄的关系

The relationship between serum lipid control and restenosis after coronary stenting

目的探讨血脂控制水平与经皮冠状动脉介入治疗(PCI)术后支架内再狭窄(ISR)的关系。方法选用我院2015年8月-2017年8月在我院行冠状动脉支架植入术的患者190例,所有患者均常规口服双联抗血小板药物及他汀类药物,并于术后12月再次入院复查冠状动脉造影,根据有无支架内再狭窄分为再狭窄组24例,无再狭窄组166例,同时所有患者于PCI术前及复查冠状动脉造影前均采取外周静脉血,检测血清总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平,研究其与冠脉ISR的关系。结果2组患者性别、年龄、吸烟、高血压患病率、冠心病家族史、PCI术前总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、甘油三酯水平差异无统计学意义;Logistics多因素回归分析显示,PCI术后血清总胆固醇未下降,低密度脂蛋白胆固醇未达标,与ISR呈正相关。结论PCI术后血清总胆固醇未下降,低密度脂蛋白胆固醇未达标是ISR发生的危险因素。

Objective To investigate the relationship between serum lipid control and stent restenosis (ISR) after percutaneous coronary intervention (PCI). Methods 190 patients who underwent coronary stent implantation in our hospital from August 2015 to August 2017 were selected, All patients were routinely treated with oral dual antiplatelet agents and statins,and re-admitted to hospital 12 months after surgery for re-examination of coronary angiography, There were 24 cases in the restenosis group and 166 cases in the non-restenosis group. At the same time, all of the patients in the preoperative PCI and review before coronary angiography of the peripheral venous blood is adopted, the detection of serum total cholesterol, triglycerides, high-density lipoprotein cholesterol, low density lipoprotein cholesterol levels, study its relationship with coronary ISR. Results Two groups of patients with gender, age, smoking, family history of hypertension prevalence,coronary heart disease,PCI preoperative total cholesterol, low density lipoprotein cholesterol, high-density lipoprotein cholesterol, triglyceride level difference is no statistical significance.Logistic multivariate regression analysis showed that serum total cholesterol did not decrease after PCI, and LDL cholesterol did not reach the standard, which was positively correlated with ISR. Conclusion Serum total cholesterol did not decrease after PCI, and low density lipoprotein cholesterol was not up to standard, which was a risk factor for ISR.
