The performance of abdomen ultrasound of AIP

目的:探讨自身免疫性胰腺炎(AIP)患者经腹部超声图像特征。方法:回顾性分析在医院确诊并治疗的13例AIP患者的常规超声及1例患者的超声造影表现,同时观察患者胰腺外器官AIP相关表现,包括腮腺、颌下腺回声及胆管情况。结果:在13例AIP患者中7例(占53.8%)常规超声表现为弥漫性病变,其余6例(占46.2%)为局灶性;13例患者中9例(占69.2%)胰周出现条带样回声增强组织。局灶性病变均表现为肿块长轴与胰管走形相平行。1例超声造影表现为慢进低增强,病灶内无充盈缺损区。胆总管扩张(11例)及涎腺弥漫性病变(4例)等胰腺外AIP表现亦较为常见。结论:AIP的超声表现具有一定的特征,可作为临床诊断AIP的一项重要参考依据。

Objective: To investigate the imaging features of abdomen ultrasound of patients with autoimmune pancreatitis (AIP). Methods: The performances of routine ultrasound of 13 patients with AIP and that of ultrasound contrast of 1 patient with AIP, all of them were confirmed and treated in hospital, were retrospectively analyzed. At the same time, the relevant performances of AIP of extra-pancreatic organ, included echoes of parotid gland and salivary gland and situation of bile duct, of patients also were observed. Results: In 13 patients with AIP, the performances of routine ultrasound of 7 cases (53.8%) were diffuse lesion, and other 6 cases (46.2%) were focal lesion. And 9 cases (69.2%) of 13 patients appeared echo enhancement tissue of stripe shape around pancreas. The performance of focal lesion was long axis of lump paralleled the direction of pancreatic duct. And the performance of ultrasound contrast of 1 case was that slowly entered and hypo-enhancement, and there was no filling-defect within lesion. Besides, the performances of AIP extra-pancreas, included choledochectasia (11 cases) and diffuse lesion of salivary gland (4 cases) and others, also were relatively common. Conclusion: The ultrasound performance of AIP has a certain features, and it can be used as an important reference basis of the clinical diagnosis for AIP.
