品管圈在降低我院住院药房调剂踪近差错率中的应用

Application of Quality control circle in reducing Dispensing Errors in inpatient Pharmacy

目的:通过品管圈活动降低住院药房调剂踪近差错率,提高住院药房调配准确率,保证用药安全。方法:2017年7月~2018年6月,以“降低调剂踪近差错率”为主题,在我院住院药房实施品管圈活动,通过柏拉图、二八法则确定改善重点,通过鱼骨图找出要因并实施对策,评价实施效果。结果:有形成果住院药房平均每月发生的踪近差错率由改善前的1.54‰降低到改善后的0.57‰;无形成果住院药师的专业知识和团队凝聚力等得到很大提升;制定了住院药房标准化文件。结论:品管圈活动有效降低了住院药房调剂踪近差错率,可行有效,值得推广。

Objective:To reduce the dispensing errors of provisioning in inpatient pharmacy,to improve the accuracy of provisioning in inpatient pharmacy,and ensure the safety of patients through quality control activities. Methods:The quality control circle was applied in the inpatient pharmacy of with the theme of “reducing the dispensing errors”from July 2017 to June 2018. Improvement focus was determined through Plato and the 28's laws, the main cause was found out through the fishbone diagram ,the corresponding countermeasures were implemented. Finally,evaluate the implementation effect of the activity. Results:Tangible achievements,The average monthly error rate of inpatient pharmacy decreased from 1.54‰to 0.57‰;Intangible outcomes,the professional knowledge and team cohesion of the pharmacists in inpatient pharmacy have been greatly improved;the standardized documentation for the inpatient pharmacy is developed. Conclusion:The quality control circle activity has effectively reduced the dispensing errors of inpatient pharmacy,which is feasible and effective and worth promoting.
