北京某医院30~60岁体检人群体质指数与高尿酸血症的剂量-反应关系 被引量:3

A dose-response analysis on the association of body mass index and hyperuricemia among physical examination aged 30-60 adults in a hospital in Beijing

摘要 目的探究体质指数(BMI)与高尿酸血症(HUA)患病关联强度的剂量-反应关系。方法选取2012年在北京电力医院就诊的30~60岁的8866名体检者进行问卷调查。多因素logistic回归模型中引入限制性立方样条探讨BMI与HUA的管理程度及剂量-反应关系分析。结果8866名体检者中,男性5623名(63.4%),女性3243名(36.6%);共有1629例HUA患者(18.4%),其中男性患病率为24.9%(1399/5623),女性患病率为7.1%(230/3243)。HUA组和非HUA组的年龄、性别、吸烟、体质指数、血压、血糖和血脂等均有显著差异(均为P<0.05)。调整年龄、吸烟、收缩压、舒张压、血糖、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇后,多因素logistic回归结果显示超重男性(OR=1.68,95%CI:1.42~1.99)、肥胖男性(OR=2.16,95%CI:1.76~2.65)、超重女性(OR=1.59,95%CI:1.15~2.20)和肥胖女性(OR=2.77,95%CI:1.79~4.27)均有患HUA的风险(均为P<0.0001)。限制性立方样条分析表明,男性人群中BMI连续变化与HUA的关联强度呈非线性剂量反应关系(χ^2=15.49,P=0.0004);女性中呈线性趋势(χ^2=0.57,P=0.7521)。结论BMI连续变化与HUA存在一定的关联,其中男性人群为非线性剂量反应关系,女性为线性趋势关系。 Objective To investigate the dose-response relationship between body mass index (BMI) and hyperuricemia (HUA) among aged 30-60 adults in Beijing. Methods A questionnaire survey was conducted on 8 866 adults in Beijing Electric Power Hospital in 2012.Multivariate logistic regression and restricted cubic spline methods were used to explore the relationship between BMI and hyperuricemia management and dose-response analysis. Results Among 8866 participants, there were 5623 males (63.4%) and 3 243 females (36.6%). The prevalence rate of HUA was 18.4%, and male was higher than female (24.9% vs. 7.1%). Age, sex, smoking, BMI, blood pressure (BP), glucose and serum lipid level were significantly different (all P <0.05). After adjusting for factors such as age, smoking, BP, glucose, total cholesterol, triglyceride, HDL-C and LDL-C, results showed that overweight men ( OR =1.68, 95% CI : 1.42-1.99), obesity men (OR =2.16, 95% CI : 1.76-2.65), overweight women ( OR =1.59, 95% CI : 1.15-2.20) and obesity women (OR =2.77, 95% CI : 1.79-4.27) had increased the risk of HUA (all P <0.0001). Restricted cubic spline analysis showed a non-linear dose-response relationship between the continuous change in BMI and hyperuricemia in the male population (χ^2=15.49, P =0.0004) and a linear relationship in female (χ^2=0.57, P =0.7521). Conclusions There is a certain correlation between continuous changes of BMI and HUA. A non-linear relationship is found in male and a linear relationship was found in female.
