颅脑创伤患者早期感染监测指标分析

Analysis of early infection monitoring index in patients with traumatic brain injury

目的分析全身炎症反应综合征(ASS)评分、C反应蛋白(CRP)监测、与周围血白细胞检测在颅脑创伤(TBI)患者早期感染的临床应用。方法收集2017年1月至2018年1月TBI患者80例,根据入院诊断,将80例患者划分为感染组(40例)和非感染组(40例)。感染组、非感染组患者均给予ASS评分、CRP监测、白细胞计数监测。结果感染组ASS评分(32.44±15.98)分,非感染组ASS评分(15.45±13.62)分,差异有显著性(t=8.123,P<0.05);非感染组患者CRP监测值较正常无明显改变;感染组患者CRP监测分别在1 d、3 d、5 d、7 d监测值高于非感染组,差异有显著性(P<0.05);感染组患者1 d、3 d、5 d、7 d白细胞计数明显升高(P<0.05)。结论 ASS评分、CRP监测、白细胞计数对TBI患者早期感染有临床诊断价值。

Objective To analyze the clinical application of modified systemic inflammatory response syndrome score, namely, ASS score, C -reacion protein ( CRP ) monitoring, peripheral white blood cells monitoring in the early infection of patients with traumatic brain injuryc ( TBI ). Method 80 patients with TBI from January 2017 to January 2018 were collected and 80 patients were divided into infection group (40 cases ) and non infection group ( 40 cases ) according to admission diagnosis ( diagnosis standard of early infection of TBI ). Patients in infection group and non infection group ( according to guidelines for diagnosis and treatment of TBI) were monitored by ASS score, CRP monitoring and white blood cell count. Results The ASS score of the infection group was (32.44 ± 15.98 ) score ,and the ASS score of non infection group ( 15.45 土 13.62 ) was significant ( t= 8.123, P<0.05 ). The CRP monitoring value of non infection group was not significantly changed, and the monitoring values of CRP monitoring in Id, 3d, 5d and 7d were higher than those of non infection group. P<0.05, Id, 3d, 5d and 7d in the infection group increased significantly ( P<0.05 ). Conclusion ASS score, CRP monitoring and white blood cell count have clinical diagnostic value for early infection of patients with TBI.
