摘要
目的探讨血浆循环miR-328-3p是否与风湿性心脏病合并心房颤动的发生有关,并初步探讨其发病机制。方法选取2017年8月~2018年7月于湖南省郴州市第四人民医院心血管内科住院的19例风湿性心脏病合并心房颤动患者作为观察组,选取同期21例风湿性心脏病窦性心律患者作为对照组。采用qRT-PCR检测其血浆循环miR-328-3p的表达;分析血浆循环miR-328-3p相对表达量的ROC曲线;采用mirFocus和starBase等生物信息学软件对miR-328-3p进行生物信息预测。结果观察组的血浆循环miR-328-3p mRNA表达水平为0.31±0.05,低于对照组的0.69±0.03,差异有统计学意义(P<0.01)。ROC曲线分析,血浆循环miR-328-3p曲线下面积为0.9323,95%置信区间为0.8598~1.0005(P<0.01)。生物信息学预测显示,miR-328-3p的KEGG信号通路主要有细胞分裂、结合蛋白、心室形成、调控胰岛素受体信号通路、调控PI3K通路、调控Wnt通路等有关,其靶基因主要是TLE1、USP37、EN2、NRIP1、FOXO4等。结论血浆循环miR-328-3p可能参与风湿性心脏病合并心房颤动的发生,可以作为风湿性心脏病合并心房颤动的生物标志物;其有可能通过调控PI3K通路和Wnt通路而导致风湿性心脏病合并心房颤动的发生。
Objective To investigate whether plasma circulating microRNA-328-3 p is related to the occurrence of rheumatic heart disease with atrial fibrillation, and to explore its pathogenesis. Methods A total of 19 patients with rheumatic heart disease and atrial fibrillation from August 2017 to July 2018 were selected as observation group and 21 patients with sinus rhythm of rheumatic heart disease were selected as control group. The expression of plasma circulating microRNA-328-3 p was detected by qRT-PCR. The ROC curve of the relative expression of plasma circulating microRNA-328-3 p was analyzed. Bioinformatics software such as mirFocus and starBase were used to predict the bioinformatics of microRNA-328-3 p. Results The expression level of plasma circulating microRNA-328-3 p of the observation group was 0.31 ±0.05, lower than 0.69 ±0.03 in the control group, the difference was statistically significant(P<0.01). ROC curve analysis showed that the area under the plasma circulating miR-328-3 p curve was 0.9323, 95%confidence interval was 0.8598-1.0005(P<0.01). Bioinformatics predictions showed that the KEGG signaling pathways of microRNA-328-3 p were mainly related to cell division, binding protein, ventricular formation, insulin receptor signaling pathway, PI3 K pathway, Wnt pathway and so on. The target genes are TLE1, STT3 A, USP37, EN2, NRIP1,FOXO4 and so on. Conclusion Plasma circulating microRNA-328-3 p may be involved in the occurrence of rheumatic heart disease with atrial fibrillation, and can be used as a biomarker of rheumatic heart disease with atrial fibrillation.It may lead to the occurrence of rheumatic heart disease with atrial fibrillation by regulating PI3 K pathway and Wnt pathway.
