输尿管镜下钬激光碎石术治疗嵌顿性输尿管结石合并炎性息肉的效果

Effect of ureteroscopic holmium laser lithotripsy in the treatment of incarcerated ureteral calculi complicated with inflammatory polyps

目的探讨输尿管镜下钬激光碎石术治疗嵌顿性输尿管结石合并炎性息肉的临床效果。方法选取2016年3月~2018年3月佛山市第五人民医院收治的72例嵌顿性输尿管结石合并炎性息肉患者作为研究对象。采用随机数字表法将72例患者分为对照组(36例)和观察组(36例)。对照组患者进行常规开放式手术治疗,观察组患者采取输尿管镜下钬激光碎石术进行治疗。比较两组患者的相关临床指标、临床疗效和术后不良反应发生情况。结果观察组患者的手术持续时间、术后恢复自主活动时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);观察组患者的术后不良反应总发生率低于对照组,差异有统计学意义(P<0.05);观察组患者的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。结论使用输尿管镜下钬激光碎石术对嵌顿性输尿管结石合并炎性息肉患者进行治疗的效果较好,该术式具有较高的临床价值和安全性。

Objective To investigate the clinical efficacy of ureteroscopic holmium laser lithotripsy in the treatment of incarcerated ureteral calculi complicated with inflammatory polyps. Methods A total of 72 patients with invasive ureteral calculi complicated with inflammatory polyps admitted to the Fifth People′ s Hospital of Foshan City from March 2016 to March 2018 were selected as subjects. The 72 patients were divided into control group(36 cases) and observation group(36 cases) by random number table method. Patients in the control group underwent routine open surgery, and patients in the observation group were treated with ureteroscopic holmium laser lithotripsy. The clinical indicators, clinical efficacy and postoperative adverse reactions of the two groups were compared. Results The duration of surgery, the time of postoperative spontaneous recovery, and the length of hospital stay were shorter in the observation group than those in the control group, the intraoperative blood loss was less than that in the control group, and the differences were statistically significant(P<0.05). The total incidence of postoperative adverse reactions was lower than that of the control group, and the difference was statistically significant(P<0.05). The total treatment efficiency of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05).Conclusion The use of ureteroscopic holmium laser lithotripsy is effective in the treatment of patients with invasive ureteral calculi complicated with inflammatory polyps. This procedure has high clinical value and safety.
