我院门诊中药饮片处方点评与分析

Review and analysis of prescriptions for Chinese medicine pieces in our hospital

目的分析中药饮片处方存在的问题,从而促进其临床合理应用,提高患者用药安全。方法随机从我院信息系统中抽取2018年1~9月1150张门诊中药饮片处方,进行分类统计分析。结果不合理处方50张,包括剂量偏大/有误8例,占16%;药证不符23例,占46%;选药不适宜4例,占8%;未标注特殊煎法5例,占10%;服用方法不适宜5例,占10%;疾病诊断太过笼统3例,占6%等,不合格率为4.35%,合格处方率为95.65%。结论我院门诊中药饮片合格率较高,但对不规范处方请医师认真对待,同时加强药师业务学习,重视处方点评工作,提高专业技术水平,切实确保患者安全、合理、有效用药。

Objective To analyze the problems existing in the prescriptions of Chinese medicine pieces, so as to promote its rational clinical application and improve the safety for patients. Methods A total of 1150 outpatient prescriptions for Chinese medicine pieces from January to September in 2018 were randomly selected from our hospital information system for statistical analysis. Results Among them, 50 prescriptions were unreasonable, including large dose/error in 8 cases, accounting for 16%;inconsistence of drug and pattern in 23, accounting for 46%;inappropriate herbal medicine selection in 4 cases, accounting for 8%;special decoction without labeling in 5 cases, accounting for 10%;improper administration in 5 cases, accounting for 10%;the disease diagnosis being too general in 3 cases, accounting for 6%. The unqualified rate was 4.35%, and the qualified prescription rate was 95.65%. Conclusion The qualified rate of Chinese medicine pieces in our outpatient clinic is relatively high, but the doctors should pay attention to the nonstandard prescriptions. At the same time, the occupational study of pharmacist should be strengthened and the prescription review should be attached great importance in order to improve the professional and technical levels and ensure patients′ safe, rational and effective medication.
