基于中医传承辅助平台对针灸治疗支气管哮喘配穴及治法特点分析

Acupoints Prescription and Treatment Characteristic of Acupuncture and Moxibustion Treating Bronchial Asthma Based on TCM Inheritance Support Platform

目的:运用中医传承辅助平台(V2.5),基于文献挖掘,分析针灸治疗支气管哮喘的配穴规律,总结针灸治疗支气管哮喘的核心穴组及新穴位处方。方法:检索1998年1月—2018年8月CNKI收录的针灸治疗支气管哮喘的临床研究文献,将其中的穴位处方录入中医传承辅助平台,采用该软件的数据分析方法,探析其中的配穴原则及规律。结果:①纳入的81篇合格文献、共84首穴位处方中使用频数最高的穴位依次为肺俞(67次)、定喘(46次)、大椎(31次)、肾俞(30次)、风门(28次)、膻中(25次)及足三里(25次)等;②设定置信度为0.6,支持度为17,得到13个常用穴位组合;③设置相关度为7、惩罚度为2,得到8个核心穴组,得到新穴位处方4首。结论:针灸治疗支气管哮喘的新穴位处方的配伍特点为“近治远治并用”,治法特点为“祛邪同时扶正,共奏阴阳平衡”,该研究得到的配穴规律、核心穴组和新穴位处方可为临床治疗支气管哮喘提供参考,新方可两两组合,交替使用,尤其适合在基层社区医院推广使用。

Objective: To analyze the law of acupoints prescription about acupuncture and moxibustion treating bronchial asthma by using TCM inheritance support platform (V2.5), and to summarize the core acupoints and new acupoint prescriptions. Methods: Clinical studies about acupuncture treating bronchial asthma from January 1998 to August 2018 were retrieved from CNKI, so as to analyze the principles and laws of the acupoints selection by using the data analysis method of the platform. Results: The acupoints with the highest frequency were BL13(67 times), EX-B1(46 times), GV14(31 times), BL23(30 times), BL12(28 times), CV17(25 times) and ST36(25 times) among the 84 pieces of acupoints prescription from the 81 included literatures. 13 common acupoints combinations were retrieved when the confidence level being 0.6 and support degree being 17.8 core acupoint groups and 4 new acupoint prescriptions were retrieved when the correlation was set to 7 and punishment set to 2. Conclusion: The new acupoint prescription has the characteristic of the combination of local points and distal points, the treatment characteristic is eliminating pathogenic factors and restoring the healthy qi simultaneously to keep the balance of yin and yang. The law of acupoint prescription, the core acupoints group, and the new acupoint prescriptions obtained in this study can provide references for clinical treatment of bronchial asthma. The new prescriptions can be used alternately in combination, which is especially suitable for the popularization in the basic community hospital.
