癌症患者疾病不确定感现状及与心理困扰相关性研究

The status of disease uncertainty in cancer patients and its correlation with psychological distress

目的探讨癌症患者疾病不确定感现状及其与心理困扰的相关性,为后续制定有效的心理干预措施提供依据。方法采用一般资料问卷、Mishel疾病不确定感量表、心理困扰温度计(DT)及问题列表(PL)对我院肿瘤内科84例住院治疗癌症患者进行调查。结果84例癌症患者疾病不确定感总分均值为(83.74±9.38),其中不明确性维度得分最高,均分为(36.20±4.74),占比85.7%患者处于中等程度的不确定感水平;疾病不确定感总分、不明确性、不可预测性维度与心理困扰得分呈正相关(r=0.265~0.544,P<0.05);疾病不确定感总分及不明确性维度与心理困扰问题列表(PL)总分及其家庭问题、情绪问题、实际问题维度得分呈正相关(r=0.262~0.441,P<0.05)。结论癌症患者疾病不确定感较强,疾病不确定感与心理困扰成正相关,在癌症患者中存在家庭问题(因照顾我而给家人带来负担)、情绪问题(担心)、实际问题(经济压力)的患者是临床护理工作中需重点关注的人群,需及时给予个性化心理护理。

Objective To explore the status of disease uncertainty in cancer patients and its correlation with mental disturbance,so as to provide basis for formulating effective psychological intervention measures.Methods The general information questionnaire,Mishel Disease Uncertainty Scale,Mental Disturbance Thermometer(DT)and Question List(PL)were used to investigate 84 hospitalized cancer patients.Results The mean score of the disease uncertainty of 84 cancer patients was(83.74±9.38),with the highest score of undefined dimension(36.20±4.74),accounting for 85.7%of the patients with moderate level of uncertainty.The total score of disease uncertainty,undefined and unpredictability dimension were positively correlated with mental disturbance score(r=0.265~0.544,P<0.05).There was a positive correlation between the total score of disease uncertainty and undefined dimension with the total score of list of psychological problems(PL),family problems,emotional problems and practical problems(r=0.262~0.441,P<0.05).Conclusion Cancer patients have a strong sense of disease uncertainty,which is positively correlated with mental disturbance.Among cancer patients,patients with family problems(burden brought to family by taking care of himself),emotional problems(worry)and practical problems(economic pressure)are the population that should be paid attention to in clinical nursing work,and personalized psychological care should be given in time.
