84例阿帕替尼治疗晚期非小细胞肺癌的临床研究

Clinical Study of Apatinib in the Treatment of Advanced Non-small Cell Lung Cancer in 84 Patients

目的:研究阿帕替尼和紫杉醇联合顺铂化疗方案治疗晚期非小细胞的临床疗效及不良反应,并进行对比分析。方法:本次研究对象为在2015年1月至2018年9月在我院住院的84例晚期非小细胞肺癌患者,按随机数字法分为观察组及对照组,每组42例患者。观察组采用阿帕替尼单药治疗,对照组采用紫杉醇联合铂类化疗,经过4周期的临床治疗之后,对两组患者的临床疗效以及不良反应进行统计分析。结果:治疗前,两组患者的年龄、性别、肿瘤类型、ECOG评分等情况均无显著差异。过4周期治疗后,观察组的疾病控制率为66.7%,显著高于对照组的40.5%,P<0.05。观察组的骨髓抑制、恶心呕吐、高血糖等不良反应发生率显著低于对照组,P<0.05。结论:阿帕替尼和紫杉醇联合铂类化疗方案相比治疗晚期非小细胞肺癌,临床疗效显著,不良反应明显减少,具有良好的有效性和安全性,适用于临床推广。

Objective: To study and compare the clinical efficacy and adverse effect of apatinib and paclitaxel plus platinum in the treatment of advanced non-small cell lung cancer. Methods: From January 2015 to September 2018, 84 patients with advanced non-small cell lung cancer selected from The General Aviation Hospital were randomly divided into the observation group and the control group. Each group contains 42 patients. The observation group was treated with apatinb and the control group was given paclitaxel combined with platinum. After 4 cycles of treatment, two groups were assessed and evaluated in terms of the efficiency and adverse effect. Results: Before treatment, there were no significantly differences in clinical data such as age, gender, tumor type and ECOG between the two groups. After 4 cycles, the disease control rate (DCR) of observation group is significantly higher than that of control group (66.7% vs 35.7%, P<0.05). The incidences of myelosuppression, nausea, vomiting and hyperglucose in observation group were all significantly lower than those in the control group (P<0.05). Conclusion: Apatinib is better than combined therapy of paclitaxel plus platinum in the treatment of advanced non-small cell lung cancer with superior efficiency and less adverse reactions and is more suitable to be used in clinic.
