激光多普勒评价温经活血方泡洗对动脉硬化闭塞症临床疗效的影响

Effect on clinical efficacy of Wenjing Huoxue recipe in the treatment of arteriosclerosis obliteration by LDF evaluation

目的激光多普勒评价温经活血方泡洗对动脉硬化闭塞症(ASO)的临床疗效及Logistic回归分析临床资料对其疗效的影响。方法选取符合纳入标准的阳虚寒凝型ASO患者80例,随机分为对照组与治疗组,每组40例;对照组予ASO常规治疗,治疗组在常规治疗基础上加用温经活血方泡洗治疗。比较两组患者治疗前后激光多普勒血流量、踝肱指数(ABI)、症状评分的变化情况,并应用多分类Logistic回归分析患者临床基线资料中影响疗效的因素。结果与本组治疗前相比,两组治疗后激光多普勒血流量升高(P<0.05),症状评分降低(P<0.05),而ABI治疗前后比较差异无统计学意义(P>0.05)。治疗组治疗后总有效率、激光多普勒血流量高于对照组(P<0.05),症状评分低于对照组(P<0.05),而ABI组间比较差异无统计学意义(P>0.05)。Logistic回归分析显示,病程、吸烟史、糖尿病史进入回归方程(OR=1.252、1.662、1.667,P<0.05),与其疗效相关。结论激光多普勒血流量能有效评价温经活血方泡洗对ASO的治疗效应,病程长、吸烟史及糖尿病史3项因素可降低其疗效。

Objective To evaluate the clinical efficacy of Wenjing Huoxue recipe in the treatment of arteriosclerosis obliteration by Laser Doppler Flowmetry(LDF)and analysis the influence of clinical data on its therapeutic effect by Logistic.Method 80 ASO with Yang deficiency and cold coagulation type cases that met the inclusion criteria were divided into two group,40 cases in each group.The control group was treated with ASO,and the treatment group was treated with foot bath with Wenjing Huoxue recipe on the basis of conventional treatment.The changes of LDF blood flow、ABI and symptom score in the two groups were observed and compared.Logistic analysis was performed to analyze the influencing factors.Results Compared with the group before treatment,the laser Doppler blood flow increased(P<0.05)and the symptom score decreased(P<0.05)in the two groups after treatment,but there was no significant difference in ABI before and after treatment(P>0.05).The total effective rate and laser Doppler blood flow in the treatment group were higher than those in the control group(P<0.05),and the symptom score was lower than that in the control group(P<0.05),but there was no significant difference in ABI between groups(P>0.05).Logistic analysis showed that the course of disease,smoking and diabetes history entered the regression equation(OR=1.252,1.662,1.667,P<0.05).Conclusions LDF can effectively evaluate the therapeutic effect of Wenjing Huoxue recipe in treatment of ASO,and the curative effect can be reduced by long course of disease,smoking and diabetes history.
