颌后入路治疗中低位髁状突骨折的体会

The Experience of Retromandibular Approach for Middle and Low Condylar Fracture

目的探索应用颌后入路治疗中低位髁突骨折的方法。方法颌后入路切口对中低位髁突骨折行复位固定术,切口在耳垂下0.5 cm处,向下延长3~3.5 cm,然后用接骨板固定髁突中低位骨折。结果骨折部位固定愈合良好,切口瘢痕不明显,面神经功能恢复,咬合功能良好,无发病率产生。结论经颌后入路切口治疗髁突骨折,是一种安全可行的方法,能有效的保护面神经,最大限度地减轻了对腮腺体的创伤,而且达到了髁突骨折内坚固复位的治疗效果。

Objective To explore the method of a retromandibular incision in the treatment of middle and low condylar fracture. Methods Retromandibular incision for the restoration and fixcation of middle and low condylar fractures. The incision is 0.5cm below the earlobe and extends 3-3.5cm downward and then use the bone fracture plate to fix middle and low condylar fractures. Results The fracture site is fixed and healed well and the incision scar is not obvious. Facial nerve functions have recovered and occlusal function is well.There is no case recurring. Conclusion Retromandibular incision for the treatment of condylar fracture is a safe and feasible method, it can protect facial nerveeffectively. Minimize trauma to the parotid glandand achieve the therapeutic effect of a solid reduction in the condyle fracture.
