摘要
为了明确 EBT3 胶片在螺旋断层治疗计划剂量验证中的应用,初步探讨不同病种及治疗方式的治疗计划剂量验证通过率的标准。首先对基于 EBT3 胶片的治疗计划剂量验证的原理和方法进行阐述,再从近三年的患者治疗计划中分类遴选出头颈(鼻咽癌) 197 例、胸部(肺癌) 45 例、盆腔(宫颈癌) 100 例和 SBRT48 例,统计并分析绝对剂量偏差及γ( 3 mm/3%)通过率。结果表明头颈(鼻咽癌)绝对剂量偏差平均值为(-0. 74 ±1. 44)%,γ通过率偏差平均值为( 94. 9 ±2. 4)%;胸部(肺癌)绝对剂量偏差平均值为(-0. 17 ±1. 46)%,γ通过率偏差平均值为( 96. 4 ± 2. 24)%;盆腔(宫颈癌)绝对剂量偏差平均值为(-0. 9 ±1. 37)%,γ通过率偏差平均值为( 90 ±3. 3)%;SBRT 绝对剂量偏差平均值为(-0. 71 ±1. 32)%,γ通过率偏差平均值为( 98. 1 ±1. 1)%。四组绝对剂量偏差及γ分析通过率相比较,绝对剂量偏差差异性不具有统计学意义( P =0. 056),γ分析通过率差异性具有统计学意义( P =0)。可见有必要根据不同病种及治疗方式确定相应的治疗计划剂量验证通过率的标准,以进一步提升精准放疗的技术水平。
In order to clarify the application of EBT3 films in dose verification of tomotherapy plan and to study the standard for the passing rate of dose verification in different diseases and treatments Firstly the principle and method of dose verification of clinical plan based on EBT3 film were described 197 cases of head and neck (naso pharyngealcancer) 45casesofchest(lungcancer) 100casesofpelvic(cervicalcancer)and48casesofSBRT were classified from the clinical plan in the last three years. the absolute dose deviation and γ pass rate were calcu lated and analyzed. The results show that the mean absolute dose deviation and γ pass rate of head and neck (naso pharyngealcancer)were(-0 74±1 44)% and(94 9±2 4)%.Themeanvalueofabsolutedosedeviationγ passrateofchest(lungcancer)were(-0 17±1 46)%and(96 4±2 24)%.Themeanvalueofabsolutedose deviationandγpassrateofpelvic(cervicalcancer)were(-0 9±1 37)% and(90±3 3)%.Themeanvalue ofabsolutedosedeviationandγpassrateofSBRTwere(-0 71±1 32)%and(98 1±1 1)%. It is necessary to confirm the standard for the passing rate of dose verification in different diseases and treatments so as to further improve the technical level of precision radiotherapy.
