外泌体减轻肝脏缺血再灌注损伤及其机制的研究进展

摘要 肝脏缺血再灌注损伤是肝移植、肝脏叶段切除术及创伤休克常见的并发症,会增加移植免疫排斥与感染导致的肝功能损伤的风险,显著增加国家及个人的经济负担,因此,如何减轻肝脏缺血再灌注损伤的难题亟待解决。随着人们对肝脏缺血再灌注损伤机制认识的加深,也采取诸多减轻肝脏缺血再灌注损伤的预处理措施,包括:缺血预处理、诱导内源性抗氧化酶的上调表达、干细胞输注疗法、抑制黏附分子及炎症因子的表达、降低肝脏细胞的自噬水平及抑制肝细胞的凋亡过程[1-2]。外泌体具有诸多特性和生物学功能,肝细胞、树突状细胞和(间充质、骨髓)干细胞来源的外泌体通过不同的作用机制减轻肝脏缺血再灌注损伤。外泌体拓展细胞的应用范围,具有潜在的临床应用价值。
