循证护理对冠状动脉旁路移植术后低氧血症的影响

Effect of Evidence-based Nursing on Hypoxemia after Coronary Artery Bypass Transplantation

目的:探讨循证护理对冠状动脉旁路移植术后患者低氧血症的影响。方法:选取120例冠脉旁路移植术患者,采用常规护理的为对照组(n=60),采用循证护理的为观察组(n=60)。比较2组戒烟情况、呼吸机使用时间、低氧血症的发生率及纠正情况。结果:观察组完全戒烟率(80.0%)高于对照组(50.0%)(P<0.05);观察组呼吸机使用时间[(21.36±10.25)h]短于对照组[(32.64±12.43)h],ICU停留时间[(2.78±1.32)d]短于对照组[(3.54±1.41)d],低氧血症发生率(11.7%)低于对照组(28.3%),差异有统计学意义(P<0.05);观察组中低氧血症患者干预24h后血氧饱和度(SpO2)[(95.13±3.94)%]、氧合指数(PaO2/FiO2)[(245.26±27.31)mmHg]均高于对照组,心率(HR)[(76.82±8.04)次/min]低于对照组,差异有统计学意义(P<0.05)。结论:循证护理能够制定科学、有效的对策预防和处理冠脉旁路移植术后低氧血症的发生,有利于患者的术后恢复。

Objective:To explore the effect of evidence-based nursing on hypoxemia in patients after coronary artery bypass grafting. Methods: 120 patients undergoing coronary artery bypass grafting were divided into the control group and the observation group. 60 cases in control group were given routine nursing while 60 cases in observation group were given evidence-based nursing. The smoking cessation,the duration of ventilator use,the incidence of hypoxemia and the correction were compared between the two groups. Results: The complete smoking cessation rate in the observation group was 80.0%,higher than that 50.0% in the control group,with statistical significance ( P <0.05);the duration of ventilator use in the observation group was [(21.36±10.25) h] shorter than that [(32.64±12.43) h] in the control group,the duration of ICU stay was [(2.78±1.32) d],shorter than [(3.54±1.41) d] in the control group,and the incidence of hypoxemia was 11.7%,lower than 28.3% in the control group,with statistical significance. After 24 hours of intervention,SpO 2,PaO 2/FiO 2 in the observation group were [(95.13 +3.94)%],[(245.26 +27.31) mmHg],higher than [(91.47±3.78)%]、[(210.18±36.82)%] in the control group,and HR was [(76.82 +8.04) times/min],lower than [(85.38±8.51) times/min] in the control group ( P <0.05). Conclusion: Evidence-based nursing can formulate scientific and effective countermeasures to prevent and treat the occurrence of hemorrhage after coronary artery bypass grafting,which is conducive to the recovery of oxygenation after coronary artery bypass grafting.
