Talin-1在食管鳞状细胞癌中的表达及其潜在的早期诊断能力

摘要 目的探讨踝蛋白-1(Talin-1)在食管鳞状细胞癌(ESCC)组织中的表达及其潜在的诊断能力。方法通过免疫组化检测75对ESCC及癌旁组织芯片中Talin-1的表达情况并分析其与临床病理特征的关系,进一步通过计算曲线下面积(AUC)评价辅助诊断效能。结果 Talin-1在ESCC组织的免疫组化染色强度、阳性染色细胞百分比及染色评分均显著高于癌旁组织(阳性细胞百分比89. 7%vs. 43. 4%,P <0. 001;染色评分2. 05 vs. 0. 73,P <0. 001);针对不同ESCC组织的阳性染色细胞百分比分析发现,Talin-1在直径<4 cm的ESCC组织中的表达水平最高(<4 cm vs. 4~6 cm vs.> 6 cm=53. 3%vs. 17. 6%vs. 18. 5%,P <0. 001),随着浸润深度由黏膜层至外膜层加深,其表达水平逐渐降低(黏膜层/黏膜下层vs.肌层vs.浆膜层vs.全层=73. 4%vs. 48. 7%vs. 17. 1%vs. 0. 0%,P <0. 001);Ⅰ期及Ⅱ期ESCC中Talin-1表达水平显著高于Ⅲ期及Ⅳ期ESCC(Ⅰ vs. Ⅱ vs. Ⅲ/Ⅳ=94. 9%vs. 55. 1%vs. 0. 0%,P<0. 001);Talin-1阳性染色细胞百分比诊断早期ESCC的AUC为0. 940;Talin-1染色评分诊断早期ESCC的AUC为0. 893。结论 Talin-1在早期ESCC组织中特异性高表达,其表达水平与肿瘤大小、浸润深度及肿瘤分期相关,是一种潜在的早期ESCC诊断标志物。 Objective To investigate the expression level and potential diagnostic value of Talin-1 in esophageal squamous cell carcinoma( ESCC). Methods The expressions of Talin-1 were detected in 75 ESCC tissues and their paired non-cancerous tissuesby immunohistochemistry( IHC). The relationship between the expression level and the clinicopathological features of the patients was analyzed. Area under curve( AUC) was used to evaluate the diagnostic value of Talin-1. Results Compared to non-cancerous tissues,the IHC staining strength,staining percentage and staining score were significantly higher in ESCC tissues( Percentage of positive cells 89. 7% vs. 43. 4%,P < 0. 001;Staining score 2. 05 vs. 0. 73,P < 0. 001). The percentage of positive cells was highest in ESCC less than 4 cm( < 4 cm vs. 4 ~ 6 cm vs. > 6 cm =53. 3% vs. 17. 6% vs. 18. 5%,P < 0. 001). As ESCC infiltrated from mucosal to the full layer,its expression level gradually decreased( Mucosa/Submucosa vs. Muscularis vs. Adventitia vs. Full layer = 73. 4% vs. 48. 7% vs. 17. 1% vs. 0. 0%,P < 0. 01). For ESCC at clinical stageⅠandⅡ,the expression level of Talin-1 was higher than those of stage Ⅲ and Ⅳ( Ⅰ vs. Ⅱ vs. Ⅲ/Ⅳ = 94. 9% vs. 55. 1% vs. 0. 0%,P< 0. 001). The AUC of IHC staining percentage to diagnose ESCC was 0. 940. The AUC of IHC staining score to diagnose ESCC was 0. 940.Conclusion The expression of Talin-1 was increased in ESCC and was associated with tumor size,infiltration depth and clinical stage. Talin-1 might serve as a potential biomarker for the diagnosis of early ESCC.
