腹腔镜手术联合亮丙瑞林对卵巢子宫内膜异位囊肿患者卵巢储备功能和生育的影响

Effect of laparoscopic surgery combined with Leuprorelin on ovarian reserve function and fertility in patients with ovarian endometriotic cyst

目的探析腹腔镜手术联合亮丙瑞林对卵巢子宫内膜异位囊肿患者卵巢储备功能和生育的影响。方法选取2016年11月~2017年11月我院收治的200例卵巢子宫内膜异位囊肿患者作为研究对象,依照动态随机法分为对照组与观察组,每组各100例。对照组行腹腔镜卵巢子宫内膜异位囊肿剥除术进行治疗,观察组在对照组基础上另加用亮丙瑞林协同治疗。比较两组治疗后的血清卵泡刺激素(FSH)、黄体生成激素(LH)及雌二醇(E2)水平、复发率、再次妊娠时间及再次妊娠率。结果观察组治疗后的FSH水平低于对照组,LH和E2水平高于对照组,差异均有统计学意义(P<0.05);观察组的复发率为5.00%,低于对照组的19.00%,差异有统计学意义(P<0.05);观察组的再次妊娠时间短于对照组,差异有统计学意义(P<0.05);观察组的再次妊娠率为77.80%,高于对照组的35.30%,差异有统计学意义(P<0.05)。结论腹腔镜卵巢子宫内膜异位囊肿切除术联合亮丙瑞林治疗卵巢子宫内膜异位囊肿,可改善患者的卵巢卵巢储备功能,提高妊娠率,缩短妊娠时间和明显降低复发率,值得临床推广应用。

Objective To explore the effect of laparoscopic surgery combined with Leuprorelin on ovarian reserve function and fertility in patients with ovarian endometriosis cyst.Methods A total of 200 patients with ovarian endometriosis cyst admitted to our hospital from November 2016 to November 2017 were selected as the research objects,they were divided into control group and observation group according to the dynamic random method,100 cases in each group.The control group was treated with laparoscopic ovarian endometriosis cyst removal,while the observation group was treated with Leuprorelin on the basis of the control group.The levels of serum follicle stimulating hormone (FSH),luteinizing hormone (LH) and estradiol (E2),recurrence rate,time of second pregnancy and rate of second pregnancy were compared between the two groups.Results After treatment,the FSH level in the observation group was lower than that in the control group,and the LH and E2 levels were higher than those in the control group,the differences were statistically significant (P<0.05).The recurrence rate of the observation group was 5.00%,which was lower than 19.00% of the control group,the difference was statistically significant (P<0.05).The second pregnancy time of the observation group was shorter than that of the control group,the difference was statistically significant (P<0.05).The second pregnancy rate of the observation group was 77.80%,which was higher than 35.30% of the control group,the difference was statistically significant (P<0.05).Conclusion Laparoscopic ovarian endometriosis cystectomy combined with Leuprorelin in the treatment of ovarian endometriosis cyst can improve ovarian reserve function,improve pregnancy rate,shorten pregnancy time and significantly reduce the recurrence rate,which is worthy of clinical application.
