银屑病患者外周血Th17、Treg细胞检测及临床意义

Detection and clinical significance of Th17 cells and Treg cells in peripheral blood of psoriasis patients

目的:探讨银屑病患者外周血Th17细胞、Treg细胞检测及临床意义。方法:选取门诊、住院患者及健康体检者共96例。其中,银屑病患者34例,一般慢性皮肤病患者30例,健康对照组32例。采用流式细胞术检测3组患者外周血Th17细胞、Treg细胞、Th17/Treg比值。结果:3组患者Th17细胞、Treg细胞、Th17/Treg比值,差异均有统计学意义(P<0.05)。通过两组t检验结果分析发现,银屑病患者Th17细胞、Treg细胞、Th17/Treg比值高于慢性皮肤病患者组及健康对照组,差异有统计学意义(P<0.05)。银屑病组患者血清中IL-10及TGF-β水平较慢性皮肤病组及健康对照组患者均有下降(P<0.05)。银屑病组患者血清中IL-17、IL-22水平较慢性皮肤病组及健康对照组患者均有升高(P<0.05)。银屑病患者外周血Th17细胞、Treg细胞水平与IL-10、TGF-β水平呈负相关(P<0.05);银屑病患者外周血Th17细胞、Treg细胞水平与IL-17、IL-22呈正相关(P<0.05)。结论:银屑病患者外周血中Th17、Treg细胞二者表达水平升高,Th17/Treg比值升高,提示Th17、Treg细胞两者可能参与银屑病的发病,而阻断两者的产生途径可能成为治疗银屑病的新方法。

Objective:To investigate the detection and clinical significance of Th17 cells and Treg cells in peripheral blood of psoriasis patients.Methods:96 outpatients,inpatients and healthy people were collected,among them 34 cases were psoriasis,30 cases were general chronic dermatosis,and 32 cases were healthy controls.Peripheral blood Th17 cells,Treg cells and Th17/Treg ratios were detected by flow cytometry in three groups.Results:The variance test of the ratio of Th17 cells,Treg cells and Th17/Treg in the three groups showed significant difference ( P <0.05).Through the analysis of t test results of two groups,it was found that Th17 cells,Treg cells and Th17/Treg ratio in psoriasis patients were higher than those in general chronic dermatosis patients and healthy control group,and the difference was statistically significant ( P < 0.05).The serum levels of IL-10 and TGF-beta in patients with psoriasis were lower than those in patients with chronic dermatosis and healthy control group ( P <0.05).The serum levels of IL-17 and IL-22 in patients with psoriasis were higher than those in patients with chronic dermatosis and healthy control group ( P <0.05).The levels of Th17 cells and Treg cells in peripheral blood of patients with psoriasis were negatively correlated with IL-10 and TGF-β levels ( P <0.05).The levels of Th17 cells and Treg cells in peripheral blood of patients with psoriasis were positive with IL-17 and IL-22 ( P <0.05).Conclusion:The expression of Th17 and Treg cells in peripheral blood of patients with psoriasis is elevated,and the ratio of Th17 to Treg is elevated,suggesting that both Th17 and Treg cells may be involved in the pathogenesis of psoriasis,and blocking the pathway of their production may be a new method for the treatment of psoriasis.
