闭经患者的中医证型与性六项及AMH的相关性研究

Correlation of the TCM Syndromes,Six Sex Hormones and AMH in Amenorrhea Patients

目的探讨闭经患者的中医证型与性六项及抗苗勒管激素(AMH)水平的相关性。方法搜集2017年3月~2018年6月因备孕需求在某院体检或就诊的患者458例,根据主诉将患者分为闭经和非闭经两组,对所有患者的中医证型进行辨证,采用电化学发光法检测两组患者雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、催乳素(PRL)、孕酮(PROG)及睾酮(T)的含量,酶联免疫吸附(ELISA)法检测两组患者血清AMH浓度。结果影响两组患者的主要中医证型为肝肾阴虚;闭经组患者性激素E2、PRL及AMH含量均明显低于非闭经组(P<0.05),性激素FSH、LH含量明显高于非闭经组(P<0.05);闭经患者各中医证型间性激素E2、LH、PRL及AMH含量不同(P<0.05);闭经患者不同中医证型与FSH和LH含量呈正相关,与E2、PRL及AMH含量呈负相关,与PROG和T含量无相关性。结论闭经患者的中医证型与性六项及AMH有一定的相关性,可为闭经患者的中医治疗提供理论依据。

OBJECTIVE To explore the correlation among TCM syndromes of amenorrhea patients,six sex hormones and antiMullerian hormone( AMH) levels. RESULTS Between March 2017 and June 2018,458 patients who came to the hospital for medical examination or treatment were selected. According to the main complaint,the patients were divided into two groups: amenorrhea and non-amenorrhea,and the TCM syndromes of all patients were differentiated,the levels of estradiol( E2),follicle stimulating hormone( FSH),luteinizing hormone( LH),prolactin( PRL),progesterone( PROG) and testosterone( T) were detected by electrochemiluminescence( ECL),and the serum AMH levels were measured by ELISA. RESULTS The main TCM syndromes affecting the two groups of patients were liver and kidney yin deficiency. The levels of sex hormones E2,PRL and AMH in the amenorrhea group were significantly lower than the non-amenorrhea group( P < 0. 05),the levels of sex hormones FSH and LH were significantly higher than the non-amenorrhea group( P<0.05). The contents of sex hormones E2,LH,PRL and AMH in different TCM syndromes of amenorrhea patients were different( P<0.05). Different TCM syndromes of amenorrhea patients were positively correlated with FSH and LH content,negatively correlated with E2,PRL and AMH content,and had no correlation with PROG and T content.CONCLUSION The TCM syndrome type of amenorrhea patients has a certain correlation with serum sex hormones and AMH,which can provide a theoretical basis for TCM treatment of amenorrhea patients.
