摘要
目的探讨哮喘预测指数(API)对婴幼儿喘息治疗的应用价值。方法选取2016年7月—2018年7月住院的80例6个月~36个月的喘息患儿,依据API将其分成API阳性40例;API阴性40例;再将API阳性患儿随机分为A组20例及B组20例;API阴性患儿随机分为C组20例及D组20例。B组及C组患儿在喘息发作控制后口服孟鲁司特片,每日1次,治疗3个月;A组患儿在喘息发作控制后口服孟鲁司特片联合布地奈德雾化吸入,每日1次,治疗3个月;D组不作处理。观察6个月内4组患儿的临床床疗效及症状改善情况。结果4组患儿随访6个月,A、B2组临床控制率分别为85%,55%,差异具有统计学意义(P<0.05);C、D2组临床控制率分别为75%,65%,差异无统计学意义(P>0.05)。治疗前4组患儿日间症状以及夜间症状评分均无显著差异(P>0.05);治疗3个月、6个月后A组患儿日间及夜间症状评分低于B组(P<0.05);而C组患儿日间及夜间症状评分与D组比较无统计学差异(P>0.05)。结论API阳性患儿联合应用孟鲁司特口服与布地奈德雾化较单独使用孟鲁司特口服干预效果显著,哮喘预测指数对婴幼儿喘息的治疗具有指导作用。
Objective To explore the application value of asthma prediction index(API)in the treatment of asthma in infants.Methods Eighty asthmatic children hospitalized from July 2016 to July 2018 were divided into 40 API positive cases and 40 API negative cases according to API. Again,API positive children were randomly divided into group A (20 cases)and group B (20 cases);API negative children were randomly divided into group C (20 cases)and group D (20 cases). The children in group B and C took montelukast tablets orally once a day for 3 months after the control of asthma attack;the children in group A took montelukast tablets combined with Budesonide aerosol inhalation once a day for 3 months after the control of asthma attack;the children in group D were not treated. The clinical effects and symptoms of four groups of children were observed within 6 months.Results ①The clinical control rates of group A and group B were 85% and 55% respectively after 6 months'follow-up,with significant difference between the two groups (P<0.05),and those of group C and D were 75% and 65%,respectively,with no significant difference between the two groups(P>0.05);② There was no significant difference in the scores of daytime and nighttime symptoms between the four groups before treatment (P>0.05);the scores of daytime and nighttime symptoms in group A were lower than those in group B after 3 months and 6 months of treatment(P<0.05);while there was no difference in the scores of daytime and nighttime symptoms between group C and group D (P>0.05).Conclusion The effect of Montelukast Combined with budesonide atomization on API-positive children was remarkable,and the asthma prediction index could guide the treatment of asthma in infants.
