超声骨刀在口腔种植上颌窦底侧壁开窗提升术中的应用 被引量:4

摘要 目的探讨超声骨刀在上颌后牙区种植行上颌窦底侧壁开窗提升术中的应用效果。方法回顾性总结10例上颌后牙牙槽骨高度2mm^4mm种植患者,术中应用超声骨刀进行上颌窦底侧壁开窗、窦底黏膜提升,植入人工骨粉,同期或Ⅱ期植入种植体。结果10例超声骨刀上颌窦底侧壁开窗提升植骨,8例同期植入11颗种植体,2例因骨高度<2.5mm,并且操作中可疑牙槽嵴顶骨裂,6个月后再植入3颗种植体。患者术前、术后X线片对比平均提升窦底高度5.0mm(2.6mm^10.0mm)。术后随访半年~2年,X线片未发现种植体周围或植骨处阴影。结论应用超声骨刀行口腔种植上颌窦底侧壁开窗提升可精确地完成手术,不易损伤窦底黏膜和骨壁血管,术后反应轻,是较理想的手术工具。 Objective To explore the technical characteristics and application effect of maxillary sinus floor lateral wall fenestration and elevation with ultrasound bone knife implantation in maxillary posterior teeth area. Methods 10 cases of maxillary posterior alveolar bone implants with height of 2-4 mm were retrospectively summarized. During the operation,the maxillary sinus floor was fenestrated,the sinus floor mucosa was elevated,the artificial bone powder was implanted,and the implants were implanted at the same time or two stages.Results Ten cases of maxillary sinus floor lateral wall window lifting bone graft,8 cases of simultaneous implantation of 11 implants, 2 cases because of bone height <2.5 mm,and operation suspicious alveolar ridge crest cleft,6 months after implantation of 3 implants. All 10 cases underwent maxillary sinus floor mucosal elevation with lateral wall fenestration by ultrasound osteotomy. The average elevation sinus floor height was 5.0 mm(2.6-10.0 mm)before and after operation. Follow-up of 6 months to 2 years showed no shadow around implant or bone graft on X-ray. Conclusion Fenestration and elevation of maxillary sinus floor and lateral wall can be performed accurately by using ultrasound bone knife. It is not easy to damage sinus floor mucosa and bone wall vessels,and has a light reaction after operation. It is an ideal surgical tool.
