类风湿关节炎患者外周血单核细胞亚群变化及意义 被引量:1

Clinical significance of proportion changes of peripheral blood monocyte subsets in patients with rheumatoid arthritis

摘要 目的探讨类风湿关节炎(RA)患者外周血中各单核细胞亚群变化的临床意义。方法采用流式细胞术检测RA患者和正常人群外周血单核细胞总数及各亚群所占比率,并与RA疾病活动(diseaseactivityscore28,DAS28-ESR)评分进行相关性分析。结果RA患者外周血单核细胞总数以及中间型单核细胞亚群(CD14++、CD16+)比率显著高于正常对照组(P<0.05),经典型单核细胞亚群(CD14++、CD16-)及非经典型细胞亚群(CD14+、CD16++)比率无显著升高(P>0.05);中间型单核细胞亚群百分比与DAS28-ESR评分呈正相关(r=0.7693),经典型单核细胞亚群百分比与DAS28-ESR评分呈负相关(r=-0.7566),非经典型单核细胞亚群百分比与DAS28-ESR评分无相关性(r=0.2732)。结论中间型单核细胞亚群可能参与类风湿性关节炎发病,并可作为病情活动性参考指标。 Objective To investigate the clinical significance of proportion change of peripheral blood monocyte subsets in patients with rheumatoid arthritis.Methods The total monocyte number and the proportion of peripheral blood monocyte Subsets in patients with rheumatoid arthritis and normal controls were measured with flow cytometry,and make an correlation analyze with disease activity score 28(DAS28-ESR).Results The total monocyte number and the proportion of intermediate monocyte Subsets(CD14++、CD16+)in patients with rheumatoid arthritis was significantly elevated as compared to normal controls (P<0.05),but the proportion of classical (CD14++、CD16-)and non-classical monocyte subsets (CD14+、CD16++)was not significantly increased as compared to normal group (P>0.05);The proportion of intermediate monocyte Subsets has a positive correlation with DAS28-ESR (r=0.7693);and the proportion of classical monocyte subsets has a negative correlation with DAS28-ESR (r=-0.7566);the no classical monocyte subsets has no correlation with DAS28-ESR (r=0.2732).Conclusion The intermediate monocyte subsets may contribute to the pathogenesis of rheumatoid arthritis,and may act as an active marker of rheumatoid arthritis.
