双能量能谱CT诊断兔肝脏纤维化模型的研究 被引量:2

摘要 探究双能量能谱电子计算机断层扫描技术(DESCT)对兔肝纤维化的评估价值。选取60只新西兰大白兔,对照组10只,实验组50只,以四氯化碳(CCl4)与橄榄油50%浓度混合液皮下注射(0.5 ml/kg,每周两次)方式造模,分别在给药4、6、8、10、 12周随机抽取实验组10只以及对照组2只进行DESCT扫描,于检查后4 h内处死大白兔,以病理结果作为肝纤维化分期标准。利用能谱分析软件获得不同肝纤维化分期肝脏的相关参数:包括动脉期(AP)及静脉期(VP)碘值、测量部位密度大小(即CT值)及标准化碘值(NIC),并对不同肝纤维化分期的肝脏体积进行测量,采用单因素方差分析对各组结果进行统计学分析。结果表明随着肝纤维化程度的加重,AP及VP碘值、CT值及NIC均下降,碘值及NIC组间差异有统计学意义(P<0.05),各期CT值组间差异无统计学意义。S1~S2期肝脏体积较对照组肿大,S3~S4期肝脏体积缩小,且组间差异有统计学意义(P<0.05)。DESCT多模态成像技术可以观察及评估兔肝纤维化程度,并提供量化指标。 To explore the value of dual-energy spectrum computed tomography(DESCT) in the evaluation of hepatic fibrosis in rabbits.A total of 60 New Zealand white rabbits were included in this research and divided into two groups. 50 of them in the experimental group,liver fibrosis model was established by intraperitoneal injection of carbon tetrachloride(0.5 ml/kg,twice a week),the rest 10 as the control group. 10 rabbits in the experimental group and 2 in the control group were randomly selected for DESCT scanning at 4, 6, 8, 10 and 12 weeks, respectively. The rabbits were killed within 4 hours after DESCT examination, and the pathological results were used as the criteria for hepatic fibrosis staging. All data including CT value of arterial phase and portal venous phase, Iodine value and normalized iodine concentration(NIC), and liver volume of different stages of hepatic fibrosis were measured by using Gemstone Spectral Imaging(GSI) viewer. The results of each group were statistically analyzed by one-way ANOVA test. CT values,Iodine values and NIC in two phases decreased with the increasing severity of hepatic fibrosis, and the differences of Iodine values and NIC among all groups were significantly different(P<0.05), but there were no significant difference in CT value among all groups. Compared with the control group, the volume of liver in S1~S2 stage were increased, and decreased in S3~S4, and there were significant difference among all groups(P<0.05). DESCT multimodal imaging technique can be used to observe and evaluate the degree of hepatic fibrosis in rabbits, which provide quantitative indicators.
