天灸疗法治疗慢性阻塞性肺疾病稳定期中不同穴位贴敷时间的效果比较

Comparative study of acupoint sticking therapy with different application time in treatment of chronic obstructive pulmonary disease at stable stage

目的探讨天灸疗法治疗慢性阻塞性肺疾病(COPD)稳定期中不同贴敷时间的临床效果,确定最佳贴敷时间。方法将90例患者随机分为3组,各30例,A组(贴敷时间2h),B组(贴敷时间4h),C组(贴敷时间6h),选择初伏、中伏、末伏进行贴敷,共贴3次;干预前、干预结束后1个月内,评价临床症状积分、皮肤刺激评分及强度。结果干预后,3组临床症状积分较干预前显著降低(P<0.01),B、C组明显低于A组(P<0.05),但C组与B组间比较差异无统计学意义(P>0.05);干预后,A、B、C组皮肤刺激评分分别为(1.73±1.16)分、(2.00±0.83)分、(2.61±1.15)分,其中C组明显高于A、B组,差异有统计学意义(P<0.05)。按强度分类,C组皮肤刺激性强度明显高于A、B组,3组间比较差异有统计学意义(P<0.05)。结论天灸疗法穴位贴敷时间持续4h,不仅使COPD患者临床症状得到明显改善,且患者的皮肤红肿、破溃情况较少,其疗效和安全性综合效益更佳。

Objective To compare the clinical effect of acupoint sticking therapy with different application time in treatment of chronic obstructive pulmonary disease(COPD)at stable stage.Methods Totally 90 patients with COPD at stable stage were assigned to three groups,group A(n=30,with 2 h of acupoint application),group B(n=30,with 4 h of acupoint application)and group C(n=30,with 6 h of acupoint application).The acupoint sticking therapy was adopted at beginning,middle and end of dog days.The scores of clinical symptom and skin irritation were compared between three groups.Results The score of clinical symptom was decreased in three groups after the intervention(P<0.01).The score of clinical symptom in the group A was higher than that in the group B and group C(P<0.05),and there was no significant difference between group B and group C.The score of skin irritation was(1.73±1.16)in group A,(2.00±0.83)in group B and(2.61±1.15)in group C,with a significant difference(P<0.05).Patients in the group A and group B had less skin irritation severity than that in group C(P<0.05).Conclusion A four-hour acupoint application is effective to relieve clinical symptom score of COPD at stable stage,showing a better overall efficacy and safety.
