血清乳酸脱氢酶水平在霍奇金淋巴瘤与非霍奇金淋巴瘤疾病诊断中的价值

Study on the value of serum lactate dehydrogenase in diagnosis of Hodgkin lymphoma and non-Hodgkin lymphoma

目的探讨非霍奇金淋巴瘤(non-Hodgkin′s lymphoma,NHL)和霍奇金淋巴瘤(Hodgkin lymphoma,HL)患者血清乳酸脱氢酶(Lactate dehydrogenase,LDH)检测临床意义。方法统计该院2015年10月至2018年10月217例住院淋巴瘤患者(其中NHL患者196例,HL患者21例)血清LDH并对临床资料进行回顾性分析,同时选择健康体检者49例血清LDH作为对照组。结果NHL患者LDH含量为(294.47±173.45)U/L,HL患者LDH含量为(206.48±55.98)U/L,健康对照组LDH含量为(171.51±23.69)U/L。NHL患者组LDH结果高于对照组及HL患者组,均有显著性差异(均P<0.05),而HL患者组与正常对照组无显著性差异(P>0.05),HL患者和NHL患者组的LDH阳性率有显著性差异(χ^2=217,P<0.05)。NHL患者组女性与男性LDH含量无统计学差异(t=0.324,P>0.05),但女性比男性LDH阳性率高,两者有显著性差异(χ^2=196,P<0.05)。结论NHL患者组LDH结果高于HL患者组,NHL患者LDH阳性率比HL患者高。NHL患者的LDH值无性别差异,但女性的阳性率高于男性。血清LDH含量在NHL患者与HL患者辅助诊断及鉴别诊断中具有一定的临床参考价值。

Objective To investigate the clinical significance of serum lactate dehydrogenase (LDH) in patients with Hodgkin′s lymphoma (HL) and non-Hodgkin′s lymphoma (NHL).Methods From October 2015 to October 2018,217 inpatients with lymphoma (196 NHL patients and 21 HL patients) were enrolled in our hospital.The clinical data were analyzed retrospectively.49 healthy people were selected as control group.Results The LDH content in NHL patients was (294.47±173.45) U/L and that in HL patients was (206.48±55.98) U/L and that in healthy control group was (171.51±23.69) U/L.The results of LDH in NHL patients group were higher than those in control group and HL patients group,there were significant differences (all P <0.05),but there was no significant difference between HL patients group and normal control group ( P >0.05).There was significant difference in the positive rate of LDH between HL patients and NHL patients group (χ^2=217,P <0.05).There was no significant difference in LDH content between women and men in NHL group ( t =0.324,P > 0.05),but the positive rate of LDH in women was higher than that in men (χ^2=196,P <0.05).Conclusion The LDH results of NHL patients are higher than those of HL patients,and the LDH positive rate of NHL patients is higher than that of HL patients.There was no gender difference in the LDH value of NHL patients,but the positive rate of females was higher than that of men.Serum LDH level has a certain clinical reference value in assistant diagnosis and differential diagnosis of NHL patients and HL patients.
