南京某妇幼专科医院职工甲状腺功能异常及相关生化结果分析

Investigation on the results of thyroid dysfunction and the relationships among related biochemical results of hospital staff from a Women-Children Special Hospital in Nanjing

目的了解南京市某妇幼专科医院健康职工甲状腺功能紊乱的患病情况以及年龄、甲状腺激素、血糖、血脂及血尿酸相互之间的相关关系。方法分别采用罗氏Cobase602全自动化学发光免疫分析仪和贝克曼AU580全自动生化分析仪对1609例本院职工进行甲状腺激素水平和血糖、血脂和血尿酸测定,运用SPSS20.0软件包对数据进行统计学分析。结果甲状腺功能紊乱总检出率为6.15%(99/1609),其中,男性和女性甲状腺功能紊乱的检出率分别为1.75%(4/229)和6.88%(95/1380),女性是男性的3.93倍,差异有统计学意义(P<0.05)。在女性甲状腺紊乱者中临床甲亢、临床甲减、亚临床甲亢和亚临床甲减的检出率分别为0.87%(12/1380)、0.43%(6/1380)、0.22%(3/1380)和5.36%(74/1380),以亚临床甲减检出率最高;各年龄组甲状腺功能紊乱的检出率相近,无统计学差异(P>0.05);亚临床甲减组女性年龄与TSH、TC、TG和LDL-C呈显著正相关(P<0.05),与FT3和FT4呈显著负相关(P<0.05)。结论南京市某妇幼专科医院职工甲状腺功能紊乱以亚临床甲减为主,女性检出率高于男性;亚临床甲减女性患者存在脂代谢紊乱,年龄是其重要危险因子,定期体检对及早发现甲状腺疾病以及代谢紊乱具有重要意义。

Objective To investigate the situation of thyroid dysfunction and the relationships among related biochemical results of hospital staff from a Women-Children Special Hospital in Nanjing.Methods A total of 1609 subjects of hospital staff from a Women-Children Special Hospital in Nanjing were enrolled.Serum thyroid hormones as well as blood glucose,lipids and uric acid were detected by Roche Cobas e602 automatic chemiluminescence analyzer and Backman AU580 automatic biochemical analyzer,respectively.The results were analyzed by SPSS 20.0 version software.Results Among 1609 subjects,the detection rate of thyroid dysfunction was 6.15%(99/1609).Female incidence rate of thyroid dysfunction (6.88%) was significantly higher than those in males (1.75%).The detection rates of hyperthyroidism,hypothyroidism,subclinical hyperthyroidism and subclinical hypothyroidism were 0.87%,0.43%,0.22% and 5.36%,respectively.There was no significant difference of female incidence rates of thyroid dysfunction in all age groups ( P >0.05).In the female subclinical hypothyroidism group,age was significantly positively correlated with TSH,TC,TG and LDL-C ( P <0.05),and was significantly negatively correlated with FT3 and FT4 ( P <0.05).Conclusion Subclinical hypothyroidism is major thyroid dysfunctions in hospital staff from a Women-Children Special Hospital in Nanjing,with higher female incidence than male′s.Age is an important risk factor in female patients with subclinical hypothyroidism characterized with lipid metabolism dysfunction.Regular physical examinations are very necessary and valuable in early detection of thyroid diseases and related metabolic disorders.
