2型糖尿病合并脑梗塞的CT影像学特点和诊断价值

CT Imaging Features and Diagnostic Value of Type 2 Diabetes Mellitus Complicated with Cerebral Infarction

目的对2型糖尿病(T2DM)合并脑梗塞CT诊断及影像学特征探析。方法选取2017年1月—2018年12月于该院诊治65例T2DM合并脑梗塞作为研究组,以同期65例非T2DM脑梗塞患者作为对照组,均行颅脑CT检查,对比分成两组临床表现,并剖析CT诊断结果及影像学特征。结果通常测定,研究组患者的空腹血糖(FPG)、餐后2h血糖(2hPG)、甘油三脂(TG)及总胆固醇(TC)均显著高于对照组(P<0.05);CT诊断显示,研究组主要是腔隙性脑梗塞,占比58.5%,且为多发性,对照组主要是单发性脑梗塞,占比53.8%,差异有统计学意义(P<0.05);T2DM合并脑梗塞发生部位包括基底节区(69.2%)、皮层下(15.4%)、脑干(9.2%)、小脑(6.2%);病灶形态以圆形或者卵圆形为主(65.0%),其次是扇形或者不规则形(35.4%)。结论T2DM合并脑梗塞主要是腔隙性,有其CT影像学特征,能够为早期确诊提供重要依据。

Objective CT diagnosis and imaging features of type 2 diabetes mellitus (T2DM) complicated with cerebral infarction were analyzed. Methods From January 2017 to December 2018, 65 patients with T2DM complicated with cerebral infarction were treated as the study group. And 65 patients with non-T2DM cerebral infarction were selected as the control group. All patients underwent CT scan. The results were divided into two groups. And CT diagnosis results and imaging features were analyzed. Results The fasting blood glucose (FPG), postprandial 2 h blood glucose (2 hPG), triglyceride (TG) and total cholesterol (TC) were significantly higher in the study group than in the control group (P<0.05). CT diagnosis showed that the study group was mainly lacunar infarction, accounting for 58.5%, and was multiple. The control group was mainly single cerebral infarction, accounting for 53.8%, the difference was statistically significant (P<0.05). The sites of T2DM with cerebral infarction included basal ganglia (69.2%), subcortical (15.4%), brainstem (9.2%), and cerebellum (6.2%);the lesions were mainly round or oval (65.0%), followed by fan or irregular (35.4%). Conclusion T2DM complicated with cerebral infarction is mainly lacunar, which has its CT imaging characteristics and can provide an important basis for early diagnosis.
