乳腺肿瘤三维超声血流参数与病理学指标的相关性

Correlation between Three Dimensional Ultrasound Blood Flow Parameters and Pathological Index in Breast Cancer

目的探讨乳腺癌三维超声血流参数与病理学指标的相关性。方法采用回顾性、总结研究方法,研究时间为2015年7月到2018年4月,选择乳腺肿瘤患者120例作为研究对象,根据病理诊断情况分为乳腺良性肿瘤70例(良性组)与乳腺癌50例(恶性组),记录所有患者的二维超声特征与三维超声血流参数,并进行相关性分析。结果恶性组二维超声主要表现为病灶微钙化、回声衰减、侧方声影、内部回声不均匀、边界不清晰、形态不规则,与良性组对比差异有统计学意义(P <0. 05)。良性组中三维超声血流显像分型Ⅰ型、Ⅱ型、Ⅲ型、Ⅳ型分别为50例、16例、4例与0例,恶性组分别为0例、6例、20例、24例,对比差异有统计学意义(P <0. 05)。在120例患者中,直线相关分析显示乳腺癌与三维超声血流显像分型、侧方声影、回声衰减、微钙化有显著相关性(P <0. 05)。结论乳腺癌三维超声血流参数与病理学诊断有很好的相关性,可提高诊断的效果,有很好的应用价值。

Objective To investigate the correlation between three-dimensional ultrasound blood flow parameters and pathological index in breast cancer. Methods 120 cases of breast tumor confirmed by operation and pathology were selected that included 70 cases of benign breast tumor( benign group) and 50 cases of breast cancer( malignant group). The two-dimensional ultrasound characteristics and three-dimensional ultrasound blood flow parameters were analyzed and given correlation analysis.Results The two dimensional ultrasonography of malignant group were mainly characterized by irregular shape,unclear boundary,inhomogeneous echo,lateral acoustic shadow,echo attenuation and microcalcification,and compared to the benign group had statistically significantly difference( P < 0. 05). The three dimensional ultrasound flow imaging in the benign group were divided into Ⅰtype,Ⅱtype,Ⅲ type and Ⅳ type with 50,16,4 and 0 cases,and were 0,6,20,and 24 cases in the malignant group,and the difference were statistically significantly( P < 0. 05). In the 120 patients,the linear correlation analysis showed that there were significant correlation between breast cancer and three-dimensional ultrasound blood flow imaging,lateral acoustic shadow,echo attenuation,and microcalcification( P < 0. 05). Conclusion The three dimensional ultrasound blood flow parameters in the diagnosis of breast cancer has good correlation with pathological diagnosis,which can improve the diagnostic effect and has a good application value.
