摘要
目的研究肿瘤标记物癌胚抗原(CEA)、鳞状上皮细胞癌抗原(SCCA)、细胞角蛋白19片段(CYFR A21-1)联合神经元特异性烯醇化酶(NSE)检测对肺癌的诊断价值。方法选取137例肺癌患者记为肺癌组,另取肺炎患者137例记为对照组。分别比较两组患者血清CEA、SCCA、CYFRA21-1、NSE水平,同时分析四项肿瘤标记物诊断肺癌的敏感性与特异度,对比不同病理类型以及不同临床分期肺癌患者血清CEA、SCCA、CYFRA21-1、NSE水平。结果肺癌组患者血清CEA、SCCA、CYFRA21-1、NSE水平分别为(60. 12±32. 42) ng/ml、(4. 52±1. 23) ng/ml、(10. 12±7. 24) ng/ml、(13. 12±9. 27) ng/ml,均明显高于对照组的(2. 11±1. 62) ng/ml、(0. 78±0. 37) ng/ml、(1. 48±0. 80) ng/ml、(7. 12±2. 11) ng/ml,组间对比差异有统计学意义(P均<0. 05)。四项肿瘤标记物联合诊断肺癌的敏感性为83. 21%(114/137),明显高于四项肿瘤标记物单独诊断的敏感性,组间对比差异均有统计学意义(P均<0. 05);而各组特异度比较均不明显,组间对比差异无统计学意义(P均> 0. 05)。Ⅳ期患者CEA、SCCA、CYFRA21-1以及NSE水平均明显高于Ⅲ期,组间对比差异有统计学意义(P均<0. 05)。结论联合检测肿瘤标记物CEA、SCCA、CYFRA21-1、NSE可有效诊断肺癌,同时可为患者病情严重程度的判断提供指导作用。
Objective To study the value of tumor markers,carcinoembryonic antigen( CEA),squamous cell carcinoma antigen( SCCA),cytokeratin 19 fragment( CYFRA21-1) and neuron specific enolase( NSE),in the detection of lung cancer.Methods 137 cases of patients with lung cancer patients with pulmonary was benign tumor group,another 137 cases was the control group. The two groups were compared with patients with serum CEA,SCCA,CYFRA21-1,NSE level,and the sensitivity analysis of four tumor marker for diagnosis of lung cancer and specificity,comparison of different pathological types and different clinical stages of lung cancer patients serum CEA,SCCA,CYFRA21-1,NSE level. Results The serum levels of CEACA and CYFRA21-1 in patients with lung cancer were( 60. 12 ± 32. 42) ng/ml.( 4. 52 ± 1. 23) ng/ml,( 10. 12 ± 7. 24) ng/ml,( 13. 12 ±9. 27) ng/ml,which was significantly higher than the control group of( 2. 11 ± 1. 62) ng/ml,( 0. 78 ± 0. 37) ng/ml,( 1. 48 ±0. 80) ng/ml,( 7. 12 ± 2. 11) ng/ml,there was significant difference between the 2 groups( all P < 0. 05). The sensitivity of 4 tumor markers in diagnosis of lung cancer was 83. 21%( 114/137),the sensitivity was significantly higher than four tumor markers in diagnosis alone,the comparison between the two groups had significant difference( P < 0. 05). The levels of CEA,SCCA,CYFRA21-1 and NSE in patients with stage Ⅳ were significantly higher than those in stage Ⅲ( P < 0. 05). Conclusion Combined detection of tumor markers CEA,SCCA,CYFRA21-1 and NSE can effectively diagnose lung cancer,and can provide guidance for judging the severity of the disease.
