克氏针辅助复位技术治疗儿童难复性肱骨髁上骨折的临床评估

Therapeutic efficacy of Kirschner-wire-assisted reduction for irreducible supracondylar fracture of humerus

目的探讨克氏针辅助复位技术治疗难复性肱骨髁上骨折的临床疗效。方法回顾性分析2014年8月至2018年8月难复性肱骨髁上骨折病例78例,其中克氏针撬拨辅助复位的病例45例,手法反复复位33例,比较2组患者在复位时间、复位满意度、术中C-arm透视次数,骨折临床愈合时间、手术并发症、术后肘关节功能等方面差异。结果克氏针辅助复位组在手术时间、复位满意度,术中透视次数与反复手法复位组差异具有统计学意义,但在肘关节功能评分、术后并发症、骨折临床愈合时间方面差异无统计学意义。结论利用克氏针辅助复位技术治疗难复性肱骨髁上骨折可以缩短手术时间、提高复位满意度,并可减少术中C-arm透视次数,值得推广。

Objective To investigate the clinical efficacy of Kirschner-wire-assisted reduction for irreduciblesupracondylar fracture of the humerus. Methods A retrospective analysis was performed for 78 cases of irreducible supracondylar fractures of humerus treated between August 2014 and August 2018, including 45 cases treated by Kirschner wire assisted reduction and 33 by manual reduction. The reduction time, patient satisfaction about the reduction, intraoperative C-arm fluoroscopy, clinical healing time, surgical complications, and postoperative elbow joint function were compared between the two groups. Results There was statistically significant differences between the Kirschner-wire-assisted reduction group and the manual reduction group in operation time, patient satisfaction about the reduction, use of intraoperative fluoroscopy, but there was no statistical difference in the elbow function score, postoperative complications, and clinical healing time of the fracture between the two groups. Conclusion Kirschner wire assisted reduction technique in irreducible supracondylar fracture of the humerus may shorten the operation time, improve patient satisfaction about the reduction, reduce the use of intraoperative C-arm fluoroscopy, and therefore it is worth for widespread use in clinical setting.
