女性生殖道沙眼衣原体感染、HPV感染与宫颈病变的关系研究 被引量:8

Study on the relationships between Chlamydial trachomatis infection,human papillomavirus infection and cervical lesions in female reproductive tract

摘要 目的探讨女性生殖道沙眼衣原体感染、人乳头瘤病毒(HPV)感染与宫颈病变的关系.方法收集2017年1月-2018年1月就诊的153例宫颈病变患者为研究对象,分为宫颈癌组(44例)、CIN组(54例)及宫颈炎组(55例),检测3组患者沙眼衣原体及HPV感染率.结果宫颈癌组和CIN组患者沙眼衣原体感染率、HPV感染率、高危型HPV感染率、HPV混合沙眼衣原体感染率均显著高于宫颈炎组(均P<0.05),宫颈癌组患者沙眼衣原体感染率、HPV感染率、高危型HPV感染率、HPV混合沙眼衣原体感染率显著高于CIN组(P<0.05).宫颈癌组和CIN组患者单纯HPV感染率均显著低于宫颈炎组(均P<0.05),宫颈癌组患者单纯HPV感染率显著低于CIN组(P<0.05).多因素非条件Logistic回归分析显示:HPV高危型感染和HPV混合沙眼衣原体感染是宫颈病变的危险因素.结论HPV高危型感染和HPV混合沙眼衣原体感染是宫颈病变的危险因素,治疗时应及早检测HPV感染类型,有效针对微生物混合感染进行治疗,尽可能控制宫颈病变的发生、发展. Objective To explore the relationships between Chlamydial trachomatis( CT) infection,human papillomavirus( HPV) infection and cervical lesions in female reproductive tract. Methods From January 2017 to January 2018,153 patients with cervical lesions were selected and divided into cervical cancer group( 44 patients),CIN group( 54 patients),and cervicitis group( 55 patients). The infection rates of CT and HPV in the three groups were detected. Results The infection rates of CT,HPV high-risk HPV,HPV combined with CT in cervical cancer group and CIN group were statistically significantly higher than those in cervicitis group( P<0. 05). The infection rates of CT,HPV high-risk HPV,HPV combined with CT in cervical cancer group were statistically significantly higher than those in CIN group( P<0. 05). The infection rates of single HPV in cervical cancer group and CIN group were statistically significantly lower than those in cervicitis group( P<0. 05). The infection rate of single HPV in cervical cancer group was statistically significantly lower than that in CIN group( P<0. 05). Multivariate Logistic regression analysis showed that high-risk HPV infection and HPV infection combined with CT infection were risk factors of cervical lesions. Conclusion High-risk HPV infection and HPV infection combined with CT infection are risk factors of cervical lesions. Early detection of HPV infection types and effective treatment of microorganism mixed infection can control the occurrence and development of cervical lesions as much as possible.
