不同植入手术方式对微创种植牙初期稳定性的影响探究 被引量:1

摘要 目的探讨对上颌前磨牙区后天性牙缺失患者选择不同植入手术方式治疗后对其微创种植牙初期稳定性产生的影响。方法方便选择该院2016年7月-2018年9月收治的90例上颌前磨牙区后天性牙缺失患者作为实验对象;随机数表法分组后明确各组治疗方式;A1组(30例):选择常规备洞方法展开治疗;A2组(30例):选择级差备洞方法展开治疗;A3组(30例):选择骨挤压方法展开治疗;对比治疗效果。结果 A1组平均ISQ值为(53.07±5.32)N·cm,平均扭矩值为(25.99±4.05)N·cm;A2组平均ISQ值为(65.46±4.29)N·cm,平均扭矩值为(35.75±4.76)N·cm;A3组平均ISQ值为(63.81±4.39)N·cm,平均扭矩值为(32.95±4.83)N·cm;手术当日,A2组以及A3组上颌前磨牙区后天性牙缺失患者平均ISQ值以及扭矩值均明显高于A1组,差异有统计学意义(P<0.05);A2组与A3组之间在平均ISQ值以及扭矩值方面差异无统计学意义(P>0.05);术后1、2、4周,同手术当日比较,平均ISQ值均有明显降低,研究A2、A3组比研究A1组的稳定系数有明显优势,差异均有统计学意义(P<0.05);术后6W呈现出逐渐提升,8周至12周升高缓慢,研究A2组与研究A1、A3组的稳定系数比较差异无统计学意义(P>0.05)。结论上颌前磨牙区后天性牙缺失患者在接受治疗期间,级差备洞方法以及骨挤压方法的有效应用,对于患者平均ISQ值以及扭矩值改善可获得明显效果,从而对于微创种植牙初期稳定性可以做出保证,最终对于上颌前磨牙区后天性牙缺失患者的早期康复,奠定基础。 Objective To investigate the effects of different implantation methods on the initial stability of minimally invasive dental implants in patients with acquired premolars in the maxillary premolar area.Methods A total of 90 patients with congenital tooth loss in the maxillary premolar area admitted to our hospital from July 2016 to September 2018 were convenient selected as experimental subjects.The randomized number table method was used to determine the treatment methods of each group.Group A1 (30 cases): selection of the conventional hole preparation method for treatment;A2 group (30 cases): the method of selecting the differential hole preparation method was used for treatment;A3 group (30 cases): the bone extrusion method was selected for treatment;and the treatment effect was compared.Results The average ISQ value of group A1 was (53.07±5.32) N·cm,the average torque value was (25.99±4.05) N·cm;the average ISQ value of group A2 was (65.46±4.29) N·cm,and the average torque value was (35.75±4.76)N·cm;The average ISQ value of the A3 group was (63.81±4.39) N·cm,and the average torque value was (32.95±4.83) N·cm.On the day of surgery,the A2 group and the A3 group had the premolar teeth of the maxillary premolar area.The mean ISQ value and torque value of the missing patients were significantly higher than those of the A1 group,the difference was statistically significant (P<0.05).There was no statistically significant difference in the average ISQ value and torque value between the A2 group and the A3 group (P>0.05).After the operation of 1,2,4w,compared with the day of surgery,the average ISQ value was significantly reduced.The stability coefficients of the A1 and A3 groups were significantly higher than those of the A1 group (P<0.05).After 6W showed a gradual increase,8W to 12W increased slowly,there was no statistically significant difference between the study A2 group and the study A1 and A3 groups (P>0.05).Conclusion In patients with acquired premolar loss in the maxillary premolar area,the effective application of the differential hole preparation method and the bone extrusion method can significantly improve the average ISQ value and torque value of the patient,thus stabilizing the initial stage of minimally invasive implants can be guaranteed,and finally lay the foundation for the early rehabilitation of patients with acquired tooth loss in the maxillary premolar area.
