摘要
目的评价糖尿病专科护理门诊对糖尿病患者血糖控制水平、自我管理能力、生活质量的实施效果。方法2015年该院建立并开设了糖尿病护理专科门诊,选择4名具有多年糖尿病专科临床经验的专科护士出诊,针对门诊糖尿病患者开展护理评估、个性化糖尿病计划制定、健康教育、用药指导、长期追踪随访等活动。比较专科门诊建立前后糖尿病患者的血糖控制水平、生活质量与自我管理能力。结果糖尿病护理门诊可有效控制糖尿病患者血糖水平,显著提高患者生活质量与自我管理能力,差异均有统计学意义(P<0.05)。结论糖尿病专科护理门诊的建立可有效控制血糖水平与提高自我管理能力,对提高患者生活质量具有重要意义,值得在临床及其他专科进一步推广应用。
Objective To evaluate the effect of nurse-led clinics model on blood glucose control, self-management behavior and quality of life among patients with diabetes mellitus. Methods Nurse-led clinics for diabetes mellitus was established since 2015.Four clinical nursing specialists were provided professional service including systematic nursing assessment,plan making, health education,medication instrument,long term follow-up etc. Results After the intervention, the blood glucose control, self-management and quality of life were significantly improved( P <0.05). Conclusion Nurse-led clinics can improve the self-management behavior and quality of life among patients with diabetes mellitus.Therefore,it is valuable to spread to other nursing areas in the future.
