网球腿的超声诊断价值

Ultrasound diagnostic value of tennis legs

目的:探讨超声检查在评估网球腿程度和类型的临床应用价值。方法:2017年10月~2018年10月期间以小腿肿胀疼痛为临床表现的患者42例,为排除下肢深静脉血栓行超声检查。常规对下肢静脉进行全面检查,然后对小腿区双侧对比扫查,重点检查小腿三头肌内侧。自上而下行纵向、横向连续滑行扫查。观察皮下组织及肌肉组织有无血肿、积液、肌纹理是否紊乱,肌肉腱膜连接处是否不连续等。结果:42例中有9例超声检查小腿三头肌有异常,结合临床诊断为网球腿。损伤程度为:腓肠肌内侧头部分撕裂2例,腓肠肌和比目鱼肌间积血、血肿疑跖肌腱断裂3例,腓肠肌内侧头微小撕裂4例。有l例经手术治疗,余8例接受保守治疗后痊愈。结论:超声检查不仅可提示网球腿的诊断,为小腿肿胀疼痛提供一条新的思路,并可准确判断损伤程度,为临床治疗方式的选择提供帮助。

Objective: To explore the clinical value of ultrasonography in evaluating the degree and type of tennis legs. Methods: From October 2017 to October 2018,42 patients with swelling and pain of the lower leg were clinically manifested,and clinical ultrasonography was performed to eliminate deep venous thrombosis of the lower limb. Routine examination of the veins of the lower limbs. Then,the contralateral scan of the calf area was carried out,focusing on the medial side of the triceps. Run vertically and horizontally from top to bottom. To observe whether the subcutaneous tissue and muscle tissue have hematoma,fluid accumulation,muscle texture disorder,muscle aponeurosis joint discontinuity,etc. Results: Among the 42 people,9 cases were diagnosed as tennis legs by ultrasonography combined with clinical diagnosis of abnormal triceps. The degree of injury was as follows: there were 1 cases of complete head tear in the medial gastrocnemius muscle,2 cases of partial head tear in the medial gastrocnemius muscle,2 cases of hematoma and suspected metatarsal tendon rupture between the gastrocnemius muscle and the soleus muscle,and 4 cases of minor head tear in the medial gastrocnemius muscle. A total of l cases were treated by surgery,and the remaining 8 cases recovered after conservative treatment. Conclusions: Ultrasonography can not only prompt the diagnosis of tennis leg,but also provide a new way of thinking for the swelling and pain of lower leg,and accurately determine the degree of injury,so as to provide help for the selection of clinical treatment methods.
