实时三维超声心动图结合CT对冠心病左室收缩功能评价的临床价值

Clinical Value of Real-Time ThreeDimensional Echocardiography Combined with CT in Evaluating Left Ventricular Systolic Function in Patients with Coronary Heart Disease

目的研究实时三维超声心动图(RT-3DE)结合640层螺旋CT(640-MSCT)对冠心病(CHD)左室收缩功能评估价值。方法将医院2017年1月-2018年1月间收治的60例CHD患者纳为CHD组,将同期行健康体检者的60例健康人纳为健康对照组,①对比两组RT-3DE及640-MSCT检查结果;②比较CHD患者RT-34DE及640-MSCT检查结果;③分析两种检查在评估CHD患者心功能中的相关性。结果①RT-3DE及640-MSCT检查均提示CHD患者EDV、ESV、SV测量水平均显著高于健康对照组,EF检查结果显著低于健康对照组(P<0.05);②640-MSCT对CHD患者EDV、ESV及SV的检测值均高于RT-3DE,EF检测值低于RT-3DE,但两种检查方式对心功能各指标的检查结果无显著性差异(P>0.05);③RT-3DE及640-MSCT在评估CHD患者心功能指标EDV、ESV、SV及EF间均具有良好的相关性(r=0.871,0.801,0.715,0.657,P均<0.05)。结论 RT-3DE与640-MSCT均能准确评估CHD患者心功能,两种检测方法在评估心功能各指标间具有极强的相关性,但两者各有利弊,临床中应结合患者具体病情选取合适的检查方式,必要时可结合两种检查结果以提高临床影像学诊断。

Objective To study the evaluated value of real-time three-dimensional echocardiography(RT-3 DE) combined with 640-slice spiral CT(640-MSCT) in left ventricular systolic function in patients with coronary heart disease(CHD). Methods Sixty patients with CHD admitted to the hospital from January 2017 to January 2018 were enrolled in CHD group. Another 60 healthy people who underwent healthy physical examination at the same time period were included in healthy control group. The results of RT-3 DE and 640-MSCT were compared between the two groups. The results of RT-34 DE and 640-MSCT examination were compared among patients with CHD.The correlation between the two examinations in evaluating cardiac function in patients with CHD was analyzed. Results RT-3 DE and 640-MSCT examinations showed that the measurement levels of EDV, ESV and SV in CHD patients were significantly higher than those in healthy control group, and the result of EF was significantly lower than that in healthy control group(P<0.05). The detection values of EDV, ESV and SV by 640-MSCT for patients with CHD were higher than those by RT-3 DE while the detection value of EF was lower than that by RT-3 DE. However, there were no significant differences between the two examination methods in the examination results of cardiac function indexes(P>0.05). RT-3 DE and 640-MSCT had good correlations between cardiac function indexes EDV, ESV, SV and EF in patients with CHD(r=0.871, 0.801,0.715, 0.657, all P<0.05). Conclusion Both RT-3 DE and 640-MSCT can accurately evaluate the cardiac function of patients with CHD. The two examination methods have strong correlations in the evaluation of cardiac function indexes, but both have their advantages and disadvantages, and appropriate examination methods should be chosen by combination of specific conditions in clinical application, if necessary, the two examination methods can be combined to improve clinical imaging diagnosis.
