用半髋关节置换术对老年不稳定型股骨粗隆间骨折患者进行治疗的效果研究

Study on the effect of hemihip replacement in the treatment of senile patients with unstable intertrochanteric fracture

目的:探讨使用半髋关节置换术对老年不稳定型股骨粗隆间骨折患者进行治疗的临床效果。方法:将2016年4月至2017年3月在阆中市人民医院进行治疗的72例老年不稳定型股骨粗隆间骨折患者作为研究对象,并将其随机平均分为甲组(n= 36)和乙组(n=36)。为甲组患者使用动力髋螺钉内固定术进行治疗。为乙组患者使用半髋关节置换术进行治疗。然后,比较两组患者的各项临床指标、术后12个月时其髋关节功能的评分及术后并发症的发生情况。结果:与甲组患者相比,乙组患者进行手术的时间更长,其术中的出血量更多,其术后卧床的时间更短,其术后12个月时髋关节功能的评分更高,其术后并发症的总发生率更低,P<0.05。结论:使用半髋关节置换术对老年不稳定型股骨粗隆间骨折患者进行治疗的效果较好,可有效地缩短其术后卧床的时间,改善其髋关节的功能。

objective: to investigate the clinical effect of semi-hip arthroplasty in the treatment of senile patients with unstable intertrochanteric fractures. Methods: 72 elderly patients with unstable intertrochanteric fracture of femur treated in langzhong people’s hospital from April 2016 to March 2017 were selected as research objects and randomly divided into group a (n=36) and group b (n=36). Patients in group a were treated with dynamic hip screw fixation. Hemihip replacement was performed in group b. Then, the clinical indicators, hip function scores and postoperative complications of the two groups were compared. Results: compared with group a, group b had longer operative time, more intraoperative blood loss, shorter postoperative bed rest, higher hip function score at 12 months postoperatively, and lower overall incidence of postoperative complications (P < 0.05). Conclusion: hemihip arthroplasty is effective in treating unstable intertrochanteric fracture of femur in the elderly.
