无抽搐电休克治疗联合小剂量艾司洛尔对精神病患者血清性激素水平的影响

Effect of MECH combined with low-dose esmolol on serum sex hormone levels in patients with mental illness

目的探讨无抽搐电休克治疗联合小剂量艾司洛尔对精神病患者血清性激素水平的影响.方法将56例精神病患者采用随机数字表法分为实验组与对照组,每组28例.两组均给予无抽搐电休克治疗,实验组联合小剂量艾司洛尔治疗.比较麻醉前、麻醉后和电刺激后1min、3min、5min患者的收缩压、舒张压、心率、血氧饱和度水平.于治疗前和治疗后1个月末采集患者静脉血测定血清中睾酮、雌二醇和催乳素水平.结果麻醉前后两组收缩压、舒张压、心率、血氧饱和度水平比较差异无统计学意义(P>0.05);无抽搐电休克治疗后1min两组收缩压、舒张压、心率、血氧饱和度水平均显著高于治疗前(P<0.05或0.01),实验组收缩压、舒张压、心率水平显著低于对照组(P<0.05或0.01);治疗后5min两组各项指标恢复至麻醉前水平.治疗1个月末实验组男女患者睾酮、雌二醇和催乳素水平均显著高于治疗前(P<0.05或0.01);实验组男性催乳素水平与女性雌二醇水平显著低于对照组(P<0.05),实验组男性及女性其余指标水平显著高于对照组(P<0.05).结论无抽搐电休克治疗联合小剂量艾司洛尔可以显著降低治疗时患者心脏和血管产生的应激反应,刺激血清睾酮、雌二醇和催乳素水平升高,有利于疾病的治疗.

Objective To study the effect of MECH combined with low-dose esmolol on serum sex hormone levels in patients with mental illness.Methods 56 patients with mental illness were divided into experimental group and control group by random number table,28 cases in each group.MECH was given to both groups,and the experimental group was treated with low-dose esmolol.The SBP,DBP, H R and SpO2 levels of patients before anesthesia,af ter anesthesia and af ter 1,3 and 5 minutes of electrical stimulation were compared.Venous blood was collected before treatment and at the end of 1 month af ter treatment to determine the levels of TES T,E2 and PRL in serum.Results There were no significant differences in the levels of SBP,DBP,H R and SpO2 between the two groups before and af ter anesthesia ( P>0.05).The levels of SBP,DBP,HR and SpO2 in the two groups were significantly higher than those before treatment at 1 min after M ECT ( P<0.05 or 0.01).The levels of SBP,DBP and HR in the experimental group were significantly lower than those in the control group ( P<0.05 or 0.01).The indicators of the two groups returned to preanesthesia levels 5 minutes after treatment.At the end of the first month of treatment,the TEST,E2 and PRL levels of the male and female patients in the experimental group were significantly higher than those before the treatment ( P<0.05 or 0.01).The male PRL level and female E2 level in the experimental group were significantly lower than those in the control group (P<0.05),and the levels of other indicators in the experimental group were significantly higher than those in the control group ( P < 0.05 ).Conclusion MECT com bined with low-dose esmolol can significantly reduce the stress response of the heart and blood vessels in patients during treatment,and stimulate the increase of serum TEST,E2 and PRL,which is conducive to the treatment of the disease.
