摘要 目的探讨细胞学与人乳头瘤病毒高危型检测在宫颈病变筛查中的应用价值.方法回顾分析2316名接受细胞学与人乳头瘤病毒高危型检测被试的检测结果,将病理检查结果作为诊断金标准,比较两种检测方式的准确度、特异度、灵敏度等.结果2316名被试的病理检查结果显示阳性756例,宫颈癌、高度鳞状上皮内病变分别为180例、576例.人乳头瘤病毒高危型检测的灵敏度显著高于细胞学检测(P<0.01),但特异度、阴性预测值、准确度均显著低于细胞学检测(P<0.01).结论宫颈病变筛查中,细胞学检测准确度与特异度高,但灵敏度低,而人乳头瘤病毒高危型检测准确度与特异度低,但灵敏度高,可联合以上两种检测方式提高筛查率. Objective To investigate the application value of cytology and high-risk HPV detection in cervical lesion screening.Methods The detection result of 2316 patients underw ent cy tology and high-risk HPV detection were retrospectively analyzed.Pathological examination results were seen as a diagnostic gold standard,compare the accuracy,specificity,and sensitivity of the two detection methods.Results The pathological examination results of 2316 subjects show ed 756 positive cases.Cervical cancer and highly squamous intraepithelial lesions were 180 cases and 576 cases,respectively.The sensitivity of high-risk HPV detection was significantly higher than that of cy tology ( P<0.01).How ever,the specificity,negative predictive value and accuracy were significantly lower than those of the cy tological test ( P<0.01 ). Conclusion In the screening of cervical lesions,the accuracy and specificity of cytology test are high,but the sensitivity is low.How ever,high-risk HPV detection has low accuracy and specificity but high sensitivity.The screening rate can be improved by com bining the above two detection methods.
