丙种球蛋白联合布地奈德对新生儿肺炎心肌酶谱、血清炎性介质和肺功能的影响 被引量:18

Effects of budesonide combined with gamma globulin on myocardial enzymes,serum inflammatory mediators and lung function in neonates with pneumonia

摘要 目的 研究丙种球蛋白联合布地奈德对新生儿肺炎心肌酶谱、血清炎症介质和肺功能的影响.方法 选择2015年1月-2017年12月西电集团医院收治的80例新生儿肺炎患者,随机分为两组.对照组单纯雾化吸入布地奈德治疗,观察组联合静脉滴注丙种球蛋白.两组均治疗7d.比较两组的治疗效果,治疗前后的心肌酶谱、血清炎症介质和肺功能.结果 观察组的有效率明显高于对照组,差异有统计学意义(P<0.05).两组治疗后的血清肌酸激酶(CK)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)水平均明显降低,同组治疗前后比较差异有统计学意义(P<0.05);且观察组明显低于对照组,差异有统计学意义(P<0.05).两组治疗后的血清C反应蛋白(CRP)、白细胞介素(IL)-4、IL-6水平均明显降低,IL-10水平均明显升高,同组治疗前后比较差异均有统计学意义(P<0.05);且观察组明显优于对照组,组间差异有统计学意义(P<0.05).两组治疗后的第一秒最大呼气量(FEVl)、用力肺活量(FVC)、最大呼气流速峰值(PEF)均明显升高,同组治疗前后比较差异有统计学意义(P<0.05);且观察组明显高于对照组,差异有统计学意义(P<0.05).结论 丙种球蛋白联合布地奈德可以有效改善新生儿肺炎的心肌酶谱、血清炎性介质和肺功能,提高治疗效果. Objective To investigate the effect of budesonide combined with gamma globulin on myocardial enzymes,serum inflammatory mediators and lung function in neonates with pneumonia.Methods Selected 80 cases of neonates with pneumonia who were treated in our hospital from January 2015 to December 2017,divided into two groups randomly.The control group was inhaled budesonide,the observation group was treated with intravenous immunoglobulin.The therapeutic effects,myocardial enzymes,serum inflammatory mediators and lung function were compared.Results The effective rate of the observation group was significantly higher than control group (P<0.05);the serum levels of CK,LDH and CK-MB in the two groups were significantly decreased (P<0.05),and the observation group was more significant (P<0.05);the average levels of serum CRP,IL-4 and IL-6 in the two groups were significantly decreased (P<0.05),and the level of IL-10 increased significantly (P<0.05),and the observation group was more significant (P<0.05);the FEV1,FVC and PEF of the two groups increased significantly after treatment (P<0.05),and the observation group was more obvious (P<0.05).Conclusion Budesonide combined with gamma globulin can effectively improve the myocardial enzymes,serum inflammatory mediators and lung function in neonates with pneumonia,and improve the therapeutic effect.
