火针结合毫针治疗肩周炎临床研究

Clinical study of Heated Needling Combined with Filiform Needling in Treatment of Scapulohumeral Periarthritis

目的:探讨火针结合毫针治疗肩周炎的临床疗效,为临床治疗肩周炎提供安全、简便、疗效确切的治疗方法。方法:将120例符合入选标准的肩周炎患者随机分为对照组(60例)和治疗组(60例)。对照组采用毫针疗法,治疗组采用火针结合毫针疗法。毫针取肩髃、肩髎、肩贞、条口及阿是穴,手阳明经走向疼痛加三间,手少阳经走向疼痛加外关,手太阳经疼痛加后溪。火针取阿是穴。分别比较由疼痛、关节活动范围和日常生活活动能力三方面的综合评定组成的百分五级评定评分及两组的临床疗效。结果:对照组2个疗程后的疼痛程度、活动范围及日常生活能力均较治疗前有极显著性改善(P<0.01);治疗组1个疗程和2个疗程后的疼痛程度、活动范围及日常生活能力均较治疗前有极显著性改善(P<0.01);治疗后治疗组的疼痛程度、活动范围及日常生活能力均较对照组同疗程显著改善(P<0.01),百分五级评定总分也较对照组同疗程显著提高(P<0.01);2个疗程后治疗组总有效率优于对照组,疗效有极显著性差异(P<0.01)。结论:火针结合毫针治疗肩周炎疗效显著,优于单纯毫针疗法治疗肩周炎。

Objective:To explore the clinical efficacy of heated needling combined with filiform needling in treatment of scapulohumeral periarthritis,and to provide the safe,simple and effective treatment for the disease. Methods:120 patients with scapulohumeral periarthritis who met the inclusion criteria were randomly divided into the control group( n =60) and treatment group( n =60).The control group was treated with filiform needling;the treatment group was treated with heated needling combined with filiform needling.The points chosen for filiform needling were LI15,TE14,SI9,ST38 and Ashi points,the chosen points also included tender points along the meridian of Hand-Yangming plus LI3,tender points along the meridian of Hand-Shaoyang plus TE5,as well as tender points along the meridian of Hand-Taiyang plus SI3.Ashi points were chosen for the heated needling.The clinical efficacy and the five-scale rating scores consisting of comprehensive assessments of pain,joint range of activities,and activities of daily living were compared between the two groups. Results:The pain level,activity range and daily living ability of the control group were significantly improved after two courses of treatment compared to those before the treatment( P <0.01);above indexes were all improved significantly both after one course and two courses of treatment compared to those before the treatment in the treatment group( P <0.01);the improvements in the treatment group were more remarkable than those in the control group( P <0.01).The total five-scale rating scores were significantly higher in the treatment group than those in the control group after the same treatment time( P <0.01).After two courses,the treatment group was better than the control group in curative effect( P <0.01). Conclusion:Heated needling combined with filiform needling is effective for scapulohumeral periarthritis,which is significantly superior to simple filiform needling.
