摘要
目的研究Napsin A在淋巴肿瘤组织中的表达,探讨Napsin A在淋巴瘤中的临床意义。方法应用免疫组化S-P法染色,检测淋巴瘤210例(包括弥漫性大B细胞淋巴瘤(DLBCL)50例,间变性大细胞淋巴瘤(ALCL)60例,霍奇金淋巴瘤(HL)30例和外周T细胞淋巴瘤60例,其他成熟B细胞类型的淋巴瘤10例及反应性淋巴结炎30例)中Napsin A的免疫组化染色表达情况。结果在反应性淋巴结炎的淋巴组织中,Napsin A阴性表达或只有少数细胞Napsin A染色呈弱阳性。ALCL的Napsin A表达率为(20.0%,12/60),其中ALK阳性的ALCL(25.0%,10/40)表达Napsin A的比率明显高于ALK阴性的ALCL(10.0%,2/20;P=0.01)。Napsin A在弥漫大B细胞淋巴瘤表达为14.0%(7/50),霍奇金淋巴瘤中表达10.0%(3/30)和外周T细胞淋巴瘤中6.6%(4/60)表达。另外,在其他类型的淋巴瘤及组织细胞肿瘤中不能检测到Napsin A。研究结果显示,有Napsin A表达的ALCL患者倾向预后较差。结论研究证实Napsin A在一部分淋巴瘤中异常表达,尤其是ALK阳性的ALCL,在其诊断和鉴别诊断中有一定意义,但是Napsin A在淋巴瘤中的生物学意义需要更深一步的研究。
Objective:To detect the expression of Napsin A in lymphatic tissues, especially in malignant lymphoma, and explore its clinical significance. Methods: Immunohistochemical staining method was used to detect the expression of Napsin A Immunohistochemical staining method was used to detect the expression of Napsin A in lymphoma including 50 cases of diffuse large B-cell lymphomas(DLBCL), 60 cases of Anaplastic large cell lymphoma(ALCL), 30 cases of Hodgkin lymphomas(HL), 60 cases of peripheral T-cell lymphomas(PTCL)and 10 cases of other B-cell type lymphoma. 30 cases of Reactive lymph node phlogistic was immunostained for Napsin A as controls. Results In reactive lymphoid tissues, only few histiocytes were positive for Napsin A. ALK-positive ALCL most frequently expressed Napsin A at a rate(25.0%,10/40) that was significantly higher compared with ALK-negative ALCL(10.0%,2/20;P=0.01). Napsin A expression was also observed in 14.0%(7/50) of DLBCL, 10.0%(3/30) of HL, 6.6%(4/60) of PTCL. Otherwise, Napsin A was not detected in other types of lymphomas or histiocytic neoplasms. Napsin A expression in systemic ALCL was associated with a higher international prognostic index. Conclusion These results demonstrated that Napsin A is aberrantly expressed in a subset of lymphomas. The biological significance is to be further investigated.
