The Effect of CUS combined with ECG and Tei Index on the Diagnostic Efficiency of Patients with Coronary Heart Disease

摘要 目的探讨心脏超声(cardiac ultrasound,CUS)联合心电图(electrocardiogram,ECG)及Tei指数对冠心病患者诊断效能的影响。方法选取我院2015年12月至2017年12月108例疑似冠心病患者,入院后均接受CUS、ECG、Tei指数及冠状动脉造影检查,并以冠状动脉造影检查结果为“金标准”,分析本组冠心病发病情况,并比较冠心病与非冠心病CUS、ECG及Tei指数诊断情况,统计分析CUS、ECG及Tei指数单独及联合诊断冠心病效果。结果经冠状动脉造影检查证实,108例疑似冠心病患者中冠心病77例、非冠心病者31例;冠心病患者节段性室壁运动异常率(88.31%)、心电图异常率(72.73%)及Tei指数值高于非冠心病者(P<0.05);联合诊断敏感度(97.40%)、准确度(94.44%)高于CUS(85.71%、86.11%)、ECG(72.73%、78.70%)及Tei指数(79.22%、82.41%)单独诊断(P<0.05),联合诊断特异度(87.10%)与CUS、ECG及Tei指数单独诊断比较无显著差异(P>0.05)。结论采取CUS及ECG、Tei指数对冠心病予以联合诊断,可显著提高疾病诊断敏感度及准确度,最大程度避免漏诊与误诊,为临床制定有针对性的干预方案提供客观参考依据。 Objective To explore the effect of cardiac ultrasound (CUS) combined with electrocardiogram (Electrocardiogram, ECG) and Tei index on the diagnostic efficiency of patients with coronary heart disease. Methods 108 suspected coronary heart disease patients in our hospital from December 2015 to December 2017 were selected. All patients received CUS, ECG, Tei index and coronary angiography after admission. Coronary angiography was used as a "gold standard" to analyze the incidence of coronary heart disease in this group. The diagnosis of (CUS), ECG and Tei index of coronary heart disease and non coronary heart disease were compared. The CUS, ECG and Tei index separately and combined diagnosis of coronary heart disease was statistically analyzed. Results Coronary angiography confirmed that there were 77 coronary heart disease patients and 31 non coronary heart disease patients in 108 suspected coronary heart disease patients. The rate of segmental wall motion abnormality (88.31%), abnormal electrocardiogram (72.73%) and Tei index in patients with coronary heart disease were higher than those of non coronary heart disease (P <0.05). The combined diagnostic sensitivity (97.40%) and accuracy (94.44%) were higher than CUS (85.71%, 86.11%), ECG (72.73%, 78.70%), and Tei index (79.22%, 82.41%) separately (P <0.05). The diagnostic specificity (87.10%) was not significantly different from that of CUS, ECG and Tei alone (P >0.05). Conclusion The combined diagnosis of CUS and ECG and Tei index for coronary heart disease can significantly improve the sensitivity and accuracy of disease diagnosis, avoid missed diagnosis and misdiagnosis to the maximum extent, and provide objective reference for clinical formulation of targeted intervention.
