髂腹股沟微创切口接骨板内固定治疗骨盆前环骨折的临床效果及安全性分析

Clinical effect and safety analysis of ilioinguinal minimally invasive incision plate internal fixation in the treatment of anterior pelvic ring fracture

目的探讨髂腹股沟微创切口接骨板内固定治疗骨盆前环骨折的临床效果及安全性。方法选取2014年6月~2018年10月中国科学院大学深圳医院收治的42例骨盆前环骨折患者作为研究对象,按照随机抽取法分为观察组与对照组,每组各21例。对照组采用传统的切开内固定方法治疗,观察组应用髂腹股沟微创切口接骨板内固定治疗。采用majeed评分评价术后骨折的复位效果,并进行比较;比较两组的手术情况和术后并发症发生情况。结果观察组的骨折复位优良率为90.48%,明显高于对照组的71.43%,差异有统计学意义(P<0.05)。观察组的手术时间短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。观察组的术后并发症总发生率为14.29%,明显低于对照组的28.57%,差异有统计学意义(P<0.05)。结论髂腹股沟微创切口接骨板内固定治疗骨盆前环骨折的用时短,恢复效果好,术后并发症发生较少,应用的安全性高,值得在临床中推广应用。

Objective To explore the clinical effect and safety of ilioinguinal minimally invasive incision plate internal fixation in the treatment of anterior pelvic ring fracture. Methods A total of 42 patients with anterior pelvic ring fracture admitted to Shenzhen Hospital, University of Chinese Academy of Sciences, from June 2014 to October 2018 were selected as the research objects, they were randomly divided into observation group and control group, with 21 cases in each group. The control group was treated with traditional incision and internal fixation, while the observation group was treated with ilioinguinal minimally invasive incision plate internal fixation. Majeed score was used to evaluate the reduction effect of fracture after operation, and the operation conditions and complications were compared between the two groups. Results The excellent and good rate of fracture reduction in the observation group was 90.48%, which was significantly higher than 71.43% in the control group, the difference was statistically significant (P<0.05). The operation time of the observation group was shorter than that of the control group, and the amount of bleeding during the operation was less than that of the control group, the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was 14.29%, which was significantly lower than 28.57% in the control group, the difference was statistically significant (P<0.05). Conclusion Ilioinguinal minimally invasive incision plate internal fixation for the treatment of anterior pelvic ring fracture takes a short time, has good recovery effect, fewer complications after operation, and has high safety. It is worth popularizing and applying in clinic.
