摘要 目的:了解黑龙江省三级公立医院医生下沉医联体内基层医疗机构的现状与意愿,探讨影响下沉基层医疗机构意愿的因素,为提高医生参与的积极性、推动医联体发展提出建议。方法:抽取黑龙江省已开展医联体工作的7家三级公立医院的医生进行问卷调查,进行描述性统计、卡方检验及影响因素分析。结果:在被调查的839位医生中,56.5%的医生愿意下沉到基层医疗机构,参与过医联体的医生占41.6%。性别、所在医院类别、工资满意度、工作环境安全感、组织主人翁氛围对下沉意愿有一定程度的影响。结论:在选派医生下沉基层医疗机构时,要综合考虑其业务能力、实际需要以及医生自身的参与意愿,并从合理分工、营造良好组织氛围、提高工作环境安全感、内外部激励相结合等方面有效提升医生下沉基层的积极性。 Objective: To explore the status quo and willingness of doctors in the tertiary public hospitals in Heilongjiang Province going to primary health care institutions, and discuss the influencing factors affecting the willingness of doctors in the tertiary public hospitals going to primary health care institutions, so as to propound the recommendations to improve the enthusiasm of the doctors in the tertiary public hospitals and advance the development of the medical consortium. Methods: Doctors from seven tertiary public hospitals in Heilongjiang Province who have been working in medical consortiums were selected for questionnaire survey. Descriptive statistics, chi-square tests and influencing factors analysis were conducted. Results: Among the 839 doctors surveyed, 56.5% were willing to go to primary health care institutions, and 41.6% have participated in the medical consortiums. Gender, hospital category, salary satisfaction, work environment security, and organizational ownership had a certain degree of influence on the willingness to go to primary health care institutions. Conclusion:In the process of selecting doctors to go to primary health care institutions, it is necessary to comprehensively consider the professional capability, actual needs and the doctor’s own willingness to participate in, and effectively enhance the enthusiasm of the doctors to go to primary health care institutions through making the rational division of labor, creating a good organizational atmosphere, improving the sense of work environment security, and combining internal and external incentives.
