交锁髓内钉治疗四肢长骨骨折的临床价值研究

Clinical value of interlocking intramedullary nail in the treatment of long bone fractures of extremities

目的研究四肢长骨骨折患者使用交锁髓内钉疗效。方法选择本院收治四肢长骨骨折患者116例,随机分为髓内钉组和钢板组,各58例,两组术前常规准备,其中髓内钉组使用交锁髓内钉治疗,钢板组使用钢板固定治疗。观察两组手术围术期指标(切口长度、术中出血量、术后引流量、手术时间、住院时间、骨折愈合时间)、随访生活质量情况(ADL量表),并记录患者并发症发生率。结果髓内钉组切口长度、术中出血量、术后引流量、手术时间、住院时间、骨折愈合时间均低于钢板组(P<0.05);术后3、6个月,髓内钉组ADL量表评分高于钢板组(P<0.05);髓内钉组并发症发生率为6.89%,低于钢板组20.68%(P<0.05)。结论交锁髓内钉治疗四肢长骨骨折疗效确切,提高患者术后生活质量,减少相关并发症,缩短骨折愈合时间。

Objective To study the efficacy of interlocking intramedullary nail in patients with long bone fractures of extremities. Methods A total of 116 patients with long bone fractures of extremities in our hospital were selected and randomly divided into intramedullary nail group and steel plate group, with 58 cases in each group. The two groups were given routine preparation before operation, and intramedullary nail group was treated with interlocking intramedullary nail, and steel plate group was given steel plate fixation. Perioperative indexes (incision length, intraoperative blood loss, postoperative drainage, operative time, hospital stay, fracture healing time) and follow-up quality of life (ADL scale) were observed in the two groups, and the incidence rate of complications was recorded. Results The incision length, intraoperative blood loss, postoperative drainage, operative time, hospital stay and fracture healing time in intramedullary nail group were lower than those in steel plate group (P<0.05). At 3 and 6 months after operation, the ADL scale score in intramedullary nail group was higher than that in steel plate group (P<0.05). The incidence rate of complications in intramedullary nail group was lower than that in steel plate group (6.89% vs 20.68%)(P<0.05). Conclusion Interlocking intramedullary nail has exact efficacy in the treatment of long bone fractures of extremities, and it can improve the quality of life, reduce related complications and shorten the fracture healing time.
