化瘀消银汤对患者Th1、Th2细胞相关因子和血液高凝状态的影响

Effects of Huayu Xiaoyin Decoction on Th1, Th2 Cell-Related Factors and Hypercoagulable State

目的:观察化瘀消银汤对血瘀型银屑病患者血清Th1、Th2细胞相关因子水平和血液高凝状态的影响。方法:按随机数表法将196例血瘀型银屑病患者分为化瘀消银汤组(研究组98例)和紫外线治疗组(对照组98例)。治疗8周后评估疗效,于治疗前(T1)及治疗8周后(T2)检测两组患者血清Th1、Th2细胞相关因子白介素(IL)-2、干扰素-γ(IFN-γ)、IL-4、IL-10及凝血-纤溶系统指标,包括纤溶酶原激活物抑制剂-1(PAI-1)、蛋白C、抗凝血酶-Ⅲ(AT-Ⅲ)水平,比较两组患者中医症状积分差异,观察不良反应情况。结果:研究组总有效率大于对照组(P<0.05)。T2时,两组血清IL-2、IFN-γ、PAI-1水平及中医症状积分均较T1时降低,且研究组低于对照组(P<0.05);血清IL-4、IL-10、蛋白C、AT-Ⅲ水平则均较T1时升高,且研究组高于对照组(P<0.05)。两组不良反应率差异无统计学意义(P>0.05)。结论:化瘀消银汤治疗血瘀型银屑病可显著提升疗效,改善患者免疫功能和血液高凝状态,缓解中医症状,安全性良好。

Objective: To observe the effects of Huayu Xiaoyin Decoction on serum levels of Th1 and Th2 cell-related factors and hypercoagulable state in patients with psoriasis of blood stasis pattern. Methods: 196 patients with psoriasis of blood stasis pattern were randomly divided into the study group( n =98) and the control group( n =98). The study group was treated with Huayu Xiaoyin Decoction, and the control group was treated with ultraviolet therapy. After 8 weeks of treatment, the efficacy was assessed, and the serum levels of Th1 and Th2 cell-related factors, i.e. IL-2, IFN-γ, IL-4 and IL-10, as well as coagulation-fibrinolysis system indexes, such as plasminogen activator inhibitor- 1(PAI-1), protein C, antithrombin-III(AT-III) were measured in the two groups before the treatment(T1) and after 8 weeks of the treatment(T2). The TCM symptoms scores were compared between the two groups, and the adverse reactions were observed. Results: The total effective rate was higher in the study group than that in the control group( P <0.05). At T2, the serum levels of IL-2, IFN-γ, PAI-1 and TCM symptoms scores were lower in the two groups than those at T1;of which the study group was significantly lower than the control group( P <0.05). The serum levels of IL-4, IL-10, protein C and AT-III were all higher than those at T1, of which the study group was higher than the control group( P <0.05). There was no significant difference in the adverse reactions rate between the two groups( P >0.05). Conclusion: Huayu Xiaoyin Decoction can significantly improve psoriasis of blood stasis pattern, improve the immune function and blood hypercoagulable state, and relieve the TCM symptoms, and it has high safety.
