高龄冠心病住院患者临床情况分析

Clinical investigation on coronary heart disease in elderly patients

目的了解高龄(≥80岁)冠心病患者临床情况。方法回顾性分析105例高龄冠心病患者的临床情况,包括身高、体质量、合并疾病、血压、心率、糖化血红蛋白、甘油三脂、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、肌酐、尿酸、高敏C反应蛋白等以及二级预防用药情况。结果105例高龄冠心病患者中,稳定性冠心病45例(42.86%),不稳定性心绞痛32例(30.48%),非ST段抬高型心肌梗死20例(19.05%),ST段抬高型心肌梗死8例(7.62%),合并疾病中高血压79例(75.24%)、高血脂68例(64.76%)、糖尿病53例(50.48%)。血压达标率81.01%,低密度脂蛋白达标率35.24%,心率达标率16.83%,糖化血红蛋白达标率69.81%。抗血小板药物、β受体阻滞剂、血管紧张素转换酶抑制剂/血管紧张素Ⅱ受体拮抗剂以及他汀类药物的使用率分别为90.48%、53.33%、22.86%、90.48%。结论80岁以上老年冠心病住院患者冠心病二级预防药物使用率偏低,心率、低密度脂蛋白胆固醇以及糖化血红蛋白达标率偏低。

Objectives To investigate the clinical characteristics of elderly patients (≥80 years old) with coronary heart disease. Methods The clinical data of 105 patients with coronary heart disease, including height, weight, complications, blood pressure, heart rate, glycosylated hemoglobin, triglyceride, total cholesterol, low-density lipoprotein, high-density lipoprotein, serum creatinine, serum uric acid, hypersensitive C-reactive protein, and secondary prevention drugs, were retrospectively analyzed. Results Among the 105 patients , 45 (42.86%) had stable coronary artery disease, 32 (30.48%) unstable angina, 20 (19.05%) non-ST-elevation myocardial infarction, and 8 (7.62%) ST-elevation myocardial infarction;and 79 (75.24%) were complicated with hypertension, 68 (64.76%) hyperlipidemia, and 53 (50.48%) diabetes mellitus. The standard-reaching rates of blood pressure, LDL-C, heart rate, and HbA1c were 81.01%, 35.24%, 16.83%, and 69.81%. The usage rates of antiplatelet,β-blockers , ACEI/ARB, and statins were 90.48%, 53.33%, 22.86%, and 90.48%. Conclusions The use rate of secondary prevention drugs and the standard-reaching rates of heart rate, LDL-C, and HbA1c in elderly patients with coronary heart disease are still low.
