摘要
目的探讨无创分娩减痛在初产妇中应用效果影响因素,针对性提出护理对策。方法选取2017年3月至2018年12月本院住院经阴道分娩初产妇患者184例进行研究,根据患者分娩疼痛分为镇痛不良组82例和镇痛良好组102例,收集两组患者相关临床资料,分析影响无创分娩减痛在初产妇中应用效果影响因素,并针对性提出护理对策。结果单因素分析得出,年龄、文化程度、体质量指数、妊娠期合并症、心理状况在镇痛不良组和镇痛良好组患者中比较,差异均有统计学意义(均P<0.05);多因素分析得出,年龄(OR=-2.314,95%CI=1.215~9.644)、妊娠期合并症(OR=2.002,95%CI=1.205~14.154)、心理状况(OR=2.514,95%CI=1.301~14.221)为影响无创分娩减痛在初产妇中应用效果独立危险因素。结论无创分娩减痛在初产妇中应用效果影响因素较多,临床中针对性进行护理干预对减轻初产妇分娩疼痛有重要价值。
Objective To explore the influencing factors of non-invasive labor and pain reduction in primiparae, and to propose nursing strategies. Methods 184 primiparae vaginally delivered in our hospital between Mar., 2017 and Dec., 2018 as study objects. According to their labor pain, they were divided into a poor analgesic group and a good analgesic group. The clinical data of the two groups were collected;and the influencing factors affecting the application of non-invasive labor and pain in primipara were analyzed;and the nursing countermeasures were proposed. Results Univariate analysis showed that there were statistical differences in age, education level, body mass index, pregnancy comorbidity, and psychological status between the two groups (all P < 0.05). Multivariate analysis showed that age (OR=-2.314, 95% CI=1.215-9.644), pregnancy complications (OR=2.002, 95% CI=1.205-14.154), and psychological status (OR=2.514, 95% CI= 1.301-14.221) were independent risk factors for the application of non-invasive labor in reducing pain in the primiparae. Conclusion Non-invasive labor and pain reduction have many influencing factors in the application in primiparae, and targeted nursing intervention in clinical practice is of great value in reducing primiparae’s pain.
